Seeking:

Bereavement Coordinator

Full-time


Summary:


The Bereavement Coordinator is responsible for planning, implementing, and maintaining the bereavement program to meet the needs of families/caregivers from VNA Hospice of NWI for up to (1) year following the death of the individual hospice patient.

Organizational Duties

1. Demonstrates behaviors that promote and support a philosophy of excellence in customer relations, and which support the mission, vision, and values of the agency.

2. Establishes and maintains effective interpersonal relationships with internal and external customers. Functions as a team member at all times.

3. Adheres to all established policies and procedures of the agency.

4. Keeps the clinical supervisor/manager/director appraised of all issues which may have an impact on the operations of the department or agency.

5. Assists the agency in detecting, reporting, and correcting work activities that may potentially be considered inappropriate.

6. Respect patient's rights and ensure confidentiality of patient records/information.

7. Compliance with agency policies and procedures relating to patient privacy and security of patient information.

8. Participates on agency committees or task forces as assigned.

9. Functions independently and appropriately.

10. Accepts and utilizes supervision.

11. Completes daily time sheets and any required paperwork promptly to ensure compliance with agency standards.

12. Drives safely, complies with traffic regulations, and wears a seat belt when travel is required in the course of work.

13. Responds to change positively.

14. Other duties as assigned by supervisor.


Program Management

1. Assists in developing and implementing policies which govern the bereavement program activities, resources and assessment of the family/care giver's needs, the presence of any risk factors associated with the patient's death, the ability of the family/care giver to cope with grief, and appropriate plans for each level of bereavement follow-up.

2. Responsible for assuring bereavement follow-up occurs according to agency policy.

3. Implements and optimizes the computerized bereavement program.

4. Plans, implements, and supervises bereavement group events (social events, workshops, educational sessions, and support groups).

5. Facilitates support groups independently, in conjunction with others or through a designee.

6. Responsible for Quality Assurance activities and reports regarding the bereavement program and submits to Hospice Clinical Director.

7. Submits timely care plans and reports as necessary.

8. Develops and coordinates tracking systems to monitor and measure bereavement activities.

9. Serves as a liaison with other agencies dealing with grief and related areas and with agencies dealing in community education and continuing education.

10. Shares in performance of bereavement counselor or assistant functions.

11. Participates in weekly interdisciplinary team meetings (IDT).

12. Participates in intern, staff, and volunteer selection and orientation.

13. Supervises support staff and interns involved in the bereavement program.

14. Participates in the supervision of bereavement volunteers.

15. Participates in regularly scheduled memorial services.

16. Acts as chairperson for Bereavement Committee.

Qualifications/Education:

Education and/or Experience

Bachelor's degree in social work, psychology, counseling, or health-related area with clinical work experience in dealing with grief or equivalent combination of education and experience. Demonstrates ability to function as an effective counselor.


Certificates, Licenses, Registrations

Must hold and maintain a valid driver's license and current automobile insurance. Must participate and comply with annual OSHA training and annual TB testing or evaluation.
