Responsibilities
- Complete comprehensive bio-psycho-social patient assessments, and utilize relevant psychotherapeutic interventions that address roles, relationships, and resources, and their impact on the patient's healthcare
- Work closely with multiple members of the interdisciplinary team including physicians, floor staff, and case managers to optimize the outcomes for patients with behavioral health needs
- Follow the patient throughout their hospital stay, which may be in multiple areas of the hospital
- Perform initial psychosocial and ongoing patient assessment and data collection focusing on emotional, behavioral, and cognitive status
- Document all patient and family interactions in a thorough, concise, objective manner, and in a timely fashion
- Work with referral sources, families, and caseworkers in assessing patients and in thorough discharge planning
- Regularly attend and participate in department staff meetings
Qualifications
Job Requirements:
- Post graduate degree
- LPC, LMFT, LCSW, or under supervision
- Minimum of one year working in a hospital setting
