The Job
The purpose of the Complex Care Behavioral Health Specialist is to collaboratively assess, plan, implement, coordinate, monitor, and evaluate the member’s health and psychosocial needs. Focusing on members’ behavioral and mental health serves to holistically support the member and potentially prevent unwanted or unnecessary events across the continuum of care.
You Will
The Behavioral Health Specialist on our Complex Care Team will function in the following ways:
- Assess members to identify medical, physical, and psychosocial needs in order to prevent utilization and support member’s overall goals of care.
- Manage members identified with mild to severe mental illness, including “high touch” members.
- Ensure and evaluate clinical appropriateness of care plan by incorporating assessment findings against evidence-based guidelines, clinical reasoning, clinical practice guidelines and/or best practices in the community.
- Provide education and coaching surrounding behavioral and mental health therapies, interventions, and pharmacological approaches.
- Create interventions and overall plan of care for members identified with a BH/MH diagnosis through risk stratification.
- Strong collaboration with external providers, specifically with BH/MH vendors and programs.
- Enhance department and organization activities by identifying opportunities for improvement to existing processes.
- Participate in activities to support professional growth through education opportunities, reading professional publications, maintain professional networks, and participating in professional organizations.
- Comply with all regulatory and quality agency standards including: Centers for Medicare and Medicaid Services (CMS), Department of Managed Health Care (DMHC), Department of Health Services (DHS), and accreditation bodies’ standards such as the National Commission of Quality Assurance (NCQA) as it relates to care management activities.
- Actively support the achievement of SCAN’s vision, mission, and goals.
- Other duties as assigned.
Your Qualifications
Master's Degree in Social Worker required. LCSW required.
CCM Certification, preferred.
2-5 years Behavioral and/or Mental Health Case Management Experience, required.
E+ Year post-Licensure Clinical Practice, preferred.
BILINGUAL- Must be bilingual in English/Spanish. (Test will be administered to assess proficiency.)
Strong knowledge & work experience with coaching & \motivational interviewing with the ability to build strong rapport.
Travel 50+% of the time in the assigned area (Ventura County, CA) - Must have a valid driver’s license, automobile insurance and reliable transportation.
