DEFINITION
Under general supervision, assesses consumer needs, connects consumers to appropriate resources, advocates to enable consumers to receive services for which they are eligible, and monitors community programs to assure effective access and service utilization.
EXAMPLES OF DUTIES
Provides accurate and informed referral information to Dane County residents seeking Behavioral Health treatment; works closely with county and community partners to identify high need individuals who would benefit from assistance in accessing services; provides clinical case consultation as needed; develops and maintains a database of Behavioral Health Resources; makes referrals for crisis services as necessary; provides prompt and adequate follow-up services to verify referral completion or to problem-solve unforeseen barriers; serves as an intake point for certain county-funded Behavioral Health services; administers initial and ongoing functional screens to determine funding eligibility; provides in-person or virtual clinical assessments, including assessments for County-funded residential treatment for substance use disorders and for mental health case management; assesses, assigns and authorizes case management services, when appropriate, to the agency that best fits the client's assessed need.
Education, Training and Experience:
Any combination of education, training and experience equivalent to a Bachelor's degree from an accredited institution in social work or other human services field and three (3) years of full time professional experience in community-based mental health and substance use services. OR: Master's degree in social work or other human services field and one year of full time professional experience in community-based mental health and substance use services.
Licenses/Certificates:
Must have one of the following State of Wisconsin licensures prior to applying: Clinical Social Worker (LCSW), Professional Counselor (LPC), Marriage and Family Therapist (LMFT), Clinical Substance Abuse Counselor (CSAC), or Substance Abuse Counselor (SAC).
Must be able to obtain Mental Health Functional Screen certification within 60 days of hire.
Special Requirements:
Must have a valid driver's license, or access to reliable personal transportation, or equivalent. Availability to work as required occasionally during nights, weekends, and during special events. Some out of county travel, with occasional overnight travel.
Background Check Statement: Some positions may require a criminal background check which can include fingerprinting due to the nature of the job's responsibilities. Physical and Environmental Work Requirements:
Professional office work environment. Ability to see and hear within normal ranges with or without adaptive devices. Prolonged computer and phone use. Manual dexterity using fine and gross motor skills. Ability to speak and be heard by others.
Knowledge of the substance use and mental health services in Dane County and ability to assess the needs of individuals requesting or presenting for substance use, mental health, or co-occurring disorder services. Knowledge of clinical Behavioral Health principles and ability to complete clinical assessments to determine appropriate level of care. Ability to assess the effectiveness of programs and agencies serving adults with substance use and mental health needs. Ability to work collaboratively within a team, and with other agencies, professionals, consumers and their guardians. Ability to use a computer to generate reports, complete forms, and document case activity. Ability to prepare and maintain complex records and reports. Excellent written and verbal communication skills. Ability to follow oral and written instructions, autonomously meet deadlines, participate in and appropriately use supervision. Ability to engage and work with diverse identities with understanding and respect for the individuals and their differences.
