Behavioral Health Discharge Planner

• Bachelor's Degree in health-related field. Master's Degree preferred.
• LPN  or LMHC preferred.
• Experience using Excel, Word, and Outlook.
• Effective time management skills.

Below is a list of your responsibilities as a Behavioral Health Discharge Planner with YesCare: 

• Participates in the development and implementation of discharge plans for patients.
• Coordinates referrals to community treatment agencies.
• Oversees the timely completion of applications and entitlement applications.
• Includes patients in the referral process
• Participates in case conferences and team meetings
• Develops and maintains contact with community agencies.
• Creates and maintains a directory of aftercare services.
• Completes required documentation.
• Collects referral data and discharge planning statistics.
• Participates and receives supervision as assigned.
• Participates in case conferences as assigned.
• Performs other duties as assigned by management.
• Follows standard precautions using personal protective equipment as required.
• Other duties as assigned by management.
