Associate Director of Community Living& Homecare Services - CHICAGO/NORTHWEST Opportunity

The Director ofCommunity Living & Homecare Services is responsible for the development, implementation, evaluation, and day-to-day operation of the 24-hour Community Living Services of IGH. Ensures that all Community Living Services meet applicable local, state, and federal laws, licensure/certification/accreditation standards, agency policies, and procedures, and meet the needs of individuals served.

About Us:
Inspired Grace Healthcare operates Community Integrated Living Facilities (CILAs), offering safe and supportive environments for individuals with disabilities. Our CILAs house between 6-8 residents each, providing a small group setting where residents can develop essential life skills under the care of trained 24-hour staff.

Position Overview:

This position is responsible for providing leadership, direction, and training to all of home managers within the assigned service delivery area(s), direction to program operations, development of systems, and promoting a positive and optimistic environment that is responsive to the needs of the individuals served, promotes active engagement, and reflects the mission of the organization. The Assistant Director works closely with the Director of Clinical Services and administrative team to coordinate and enhance services for all individuals residing in the 24-hour CILA and Homecare programs.

Principal Duties/Responsibilities:

  • Oversee the day-to-day residential operations and provide direct supervision, coaching, and mentoring to Residential Supervisors through bi-weekly supervision meetings.
  • Complete annual performance evaluations and support Residential Supervisors in the completion of DSP evaluations to ensure strengths and challenges are discussed and create and implement program goals that support departmental outcomes.
  • Ensure homes and team members meet quality and compliance requirements as set forth by Helping Hand, DHS, IDPH, and other pertinent regulations including the fulfilment of all necessary training requirements.
  • Communicate regularly with and support Residential Supervisors to ensure continuity in areas such as but not limited to, staff scheduling, data entering and tracking, team member training and meetings, shift reports, activity planning, meal planning, inventory and budgeted shopping, safety drills, maintenance and cleanliness checks, annual evaluations, guidelines, and to monitor and offer suggestions so that team members have adequate resources with which to perform their job responsibilities.
  • Monitor and follow up regarding daily documentation, agency communication, medication needs, and home and client budgets.
  • Monitor and manage program budgets for each home location and ensure that all resources are used in a fiscally responsible manner.
  • Oversee scheduling needs per assigned home, provide direct support and shift coverage as needed, flex schedule to meet the needs of the program and monitor flexing staffing patterns to minimize overtime costs to the agency.
  • Provide input and participate as a member of the individuals interdisciplinary team as assigned, provide training and support through individualized plans, communication client individual needs in a timely and effective manner, and attend all team meetings/staffing’s as required.
  • Conduct interviews with the assistance of the Residential Supervisors and guide the process of onboarding new team members to foster positive and meaningful working partnerships.
  • Work collectively with Case Management and support team members to create and foster partnership opportunities for families and other community establishments.
  • Communicate regularly with Management on the strengths and challenges of the assigned CILA(s) and team members including scheduling, the completion of data entry, gathering information, documenting, and leading meetings associated with disciplinary actions.
  • Professionally interact with and support Residential Supervisors in addressing and responding to family/team member questions, discuss opportunities for resolution, provide solutions, and notify Director as needed.
  • Assist with, assign, and maintain flexibility while supporting community events that may occur outside of regular work hours including safely driving individuals for various programming purposes.
  • Monitor, audit, update, train, and implement tasks and documentation as assigned.
  • Other duties as assigned.
  • Keeps abreast of applicable laws, rules/regulations, and policies and procedures promulgated through code agencies responsible for surveillance and regulatory oversight of Inspired Grace Healthcare and any other entity to which the adult residential programs maintain accountability.
  • Directly responsible for hiring, onboarding, ongoing training and supervision of all home managers within their assigned service delivery area(s), conducts at least bi-weekly 1:1 supervision with supervisees, issues performance improvement plans/recommendations for discharge as needed and evaluates performance at least annually in accordance with agency policies and procedures.
  • Develops and oversees department wide system(s) including but not limited to: DSP on-boarding, completion of on the job training, logs, communication between supervisory staff and DSPs. Maintains a database for staff and managerial compliance with required trainings, including requests for medication training/cross-training, and acts as department liaison to the training department.
  • Directly responsible for ensuring all home environments within assigned service delivery area(s) are in compliance with DHS Rule 115 & 116, and licensing standards, agency mission, policy & procedure, program guidelines and best practice.
  • Reviews all quarterly environmental assessments completed by home management, maintains ongoing database of scores, reviews and maintains accountability with home management team to plans of correction for all issues identified in a timely manner, and conducts regular walkthroughs (at least monthly) of all environments assigned.
  • Maintains oversight of home supply levels and orders and makes recommendation for adjustment based on department and home resources as needed.
  • Reviews all incident reports submitted related to safety within the home environment, maintains ongoing database of incidents caused by environmental safety issues, makes immediate recommendation for and ensures follow up of corrections to environmental factors influencing life safety.
  • Monitors completion of required documentation by department staff by maintaining database of reports across assigned service delivery area, including but not limited to: unusual incident reports, body checks, progress notes, sleep, menses, and bowel elimination tracking, injury reports, etc.
  • Oversees completion and maintenance of documentation related to progressive discipline.
  • Maintains databases of staffing resources including but not limited to: department supervisory staff contacts, DSP contacts, assignment of payroll/direct supervision of DSPs, overtime, staff vacancy, call-offs & tardiness, making recommendations to improve overall efficiency. Will provided documentation for corrective actions as needed.
  • Oversees appropriate and responsible management of home and resident finances in accordance with policies/procedures, conducting SSI financial reviews, monthly. Maintains a working database of resident financial information and acts as department liaison to finance department.
  • Oversees initiation and implementation of universal precautions in home environments as deemed necessary in accordance with agency policy and procedure.
  • Responsible for providing supervisees direction related to the opening/licensing of new and current home environments, as well as coordinating resident home moves and intakes.
  • Provides 24 hour emergency coverage as needed. Participates in rotating oversight of MIC/On-Call home managers and acts as interim home manager during supervisee’s approved time off.

Other Responsibilities:

  • Participate in variety of committee and staff meeting groups.
  • Participate in quarterly quality assurance reviews.
  • Attend and assist at agency and member events.
  • Perform other duties, as assigned.

Education:

  • Bachelors (Preferred)

Experience:

  • Bachelor’s Degree required and three years of work experience in the field of developmental disabilities and mental illness.
  • Two or more years of progressively more responsible administrative and management experience. This experience in the field of human services or developmental disabilities is preferred.
  • Must be able to counsel advise, teach, resolve conflicts, and direct staff and families.
  • Must have the ability to write grant requests, action plans, and proposals to develop resources.
  • Must have the ability to manage a 12-month budget.
  • Must be able to communicate by telephone or in-person, without the assistance of adaptive equipment that another individual must operate.
  • Must be able to work a flexible schedule.
