Work Shifts SHIFT II 9:00AM-5:00PM Job Description 1PURPOSE OF POSITION: Under the supervision of the Associate Director of Risk Management for Behavioral Health, aids in planning, organizing and implementing the goals and mission of the BH Risk Management Department. The goals are to identify, evaluate and reduce or eliminate the risks of harm to all patients and staff, to facilitate the provision of a high quality of patient care through the reduction of risks of harm and to reduce the risks of financial losses to the hospital and system-wide.

SUMMARY OF ESSENTIAL DUTIES AND RESPONSIBILITIES: Review occurrence reports (VOICE) and provides analysis/summary of findings/ Resolutions Investigate events that have led to patient harm or have the potential to lead to patient harm. Review of cases for potential NYJC, NYSOMH, NYPORTS, and JACHO reportable incidents and initiate investigations and report within the designated time frames. Follow established guidelines for the reporting and closing of adverse events to the Office of Mental Health (OMH) NIMRS; New York Patient Occurrence Reporting and Tracking System (NYPORTS) Review and request video footage of cases, as necessary, and communicate with clinical departmental staff to obtain additional information regarding adverse events.

Follow up on risk reduction strategies (RRS), both implementation and effectiveness; Identify trends to be addressed with individual units/departments. Assist and Participate in Case Review Committee, SIRC (Special Incident Review Committee), RCAs and board report prep meetings as assigned. Coordinate and schedule meetings/minutes for assigned meetings.

Collaborate with Central Office Legal Affairs as assigned . Process all Notice of Claims maintain files electronically and hard copies. Maintain Notice of Claims spreadsheet data.

Process all Summons & Complaints and pre-litigates cases, maintain files electronically and hard copies. Accept and process subpoenas from process servers, Supreme Court, ADA, Criminal Court. Process request for representation to staff involved in any malpractice or tort cases.

Coordinates Conferences/WebEx, depositions and interviews of staff with outside counsel, District Attorney, the Law Department and the Supreme Court. Utilizes claims database; VOICE incident Database to access information for data collection, credentialing of King County Medical Staff, RNs NPs and PA. Create and run reports from IVOS System, VOICE system - other systems as assigned.

Track and maintain log of settled and closed cases. Sequester medical records/radiology studies/fetal strips involved in litigation or pre -litigation. Liaison with Medical Legal for certified medical records as requested by Legal Counsels/contracted Legal Counsels.

Obtain medical records for Defense Attorney's at HHC, HHC contracted law firms or Medical Litigation Unit and Sedgwick. Provide malpractice history for licensed staff to the Medical Board Office and outside Health Institutions, hospital for licensing and credentialing purposes. Perform other activities as assigned by BH Associate Director.

Minimum Qualifications 1. A Masters degree in Arts or Science with a specialization in hospital administration, health care administration, administrative medicine or in public health when conferred for a program in hospital administration, from an approved college or university; or a Baccalaureate degree; and, 2. One (1) year of administrative experience and background; or, 3.

An equivalent combination of training, education and experience in related fields and educational disciplines; and, 4. General knowledge of policies and programs utilized in the operation of a health care facility, ability to assimilate and analyze data, to recommend action based on analytical findings, and to assist in the solution of complex problems in health care administration and management.
