ABOUT THE PROGRAM:
Primary care focuses on everyday health, encompassing wellness services like physicals, vaccines and check-ups, as well as the treatment and management of common, non-emergent health concerns. Primary Care focuses on the complete and continuing healthcare of adults. Our practice serves the adult population of CHR and the adult community and offers addiction medicine services as well as primary care.
DUTIES & RESPONSIBILITIES:
- Under established guidelines and in collaboration and/or supervision with the practice physician(s), perform routine physical examinations, and formulate medical, educational, and psychosocial plans to treat patients.
- In consultation with the physician(s) and care team members, develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists.
- Provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols and identifies problems outside of their scope of practice and refers appropriately to physician, or other health team members or specialist.
- Using EMR system to improve patient outcomes, streamlined continuity of care and superior provider satisfaction.
- Take a complete medical history and do a physical examination on patients as clinically appropriate.
- Provide counseling and support as clinically appropriate, in areas of social problems recognizing the emotional component of illness, making referrals to mental health professionals as needed.
- Educate patients about disease prevention, health maintenance, and wellness.

QUALIFICATIONS:
Education: RN and Family Nurse Practitioner training program in accredited college or university required
Experience: At least 5 years of post-graduate experience as a Nurse Practitioner (APRN) within a primary care setting; MAT experience preferred.
Licensure/Certification/Registration: Active license to practice in the state of CT. Active DEA required.
Training: APRN (Family, Adult/Geri Nurse Practitioner). Must conform to prescribed standards for continuing education as set down by appropriate state or national advisory boards.

COMPENSATION*:
$121,356 /yr min to $169,898/yr max
- Actual rates are determined at the time of offer and are based on relevant experience, as well as, adherence to our internal equity policy. Many of our positions offer program/location differentials and/or sign-on incentives. (Please note, for part-time positions this range would be pro-rated based on hours worked)
