Advice Nurse Case Manager

PRIDE Health is seeking a local remote Advice Nurse Case Manager in Las Vegas, NV.

Shift: 10AM – 8:30PM

Pay rate range (based on experience) $40-42/Hour

Registered Nurse will train in the office, then work the rest of the contract remotely. Office is in Las Vegas, NV.

Job Duties:

· Assist members seek appropriate level of care as the Advice Nurse.

· Perform thorough telephonic nursing assessment and clinical review to determine appropriate triage protocol

· Assess, plan, and implement care strategies that are individualized by patient and directed toward the most appropriate, restrictive level of care.

· Utilize both company and community-based resources to establish a safe and effective case management plan for hospitalized members.

· Collaborate with member, family, and health care providers to develop and individualized plan of care that encompasses both acute care episode and post hospital discharge plan.

· Communicate with all stakeholders the required health related information to ensure quality coordinated care and services are provided expeditiously to all members.

· Advocate for members and families as needed to ensure the member’s needs and choices are fully represented and supported by the healthcare team.

Qualifications:

· 2-4 years of experience in a hospital setting with carried clinical experience.

· Case Management Certification

· Managed Care Experience

· BSN Preferred

· Active NV RN License

· InterQual knowledge

· Triage knowledge
