The Advanced Practice Provider (APP) serving Green Surgery (General Surgery - Colorectal) service is a faculty member credentialed and privileged to serve as a licensed independent practitioner and is a member of the OHSU (Oregon Health & Science University) Professional Staff practicing in accordance with the respective bylaws.

 

The APP in this role will provide medical management to inpatients on the Colorectal surgical service in the acute care units. Role includes, but is not limited to: rounding on a daily basis with team and/or faculty, developing and implementing, in conjunction with faculty physicians, clinical plans of care and/or protocols for inpatients on the Colorectal service with common conditions/diagnoses/procedures; educating the house staff and nursing unit staff on these protocols, with the goal of enhancing quality of care, enhancing quality of patient services, and decreasing length of inpatient stay; working actively with hospital and unit staff to rapidly develop and implement discharge plans for inpatients on the Colorectal service and to address length of stay issues.

 

Procedural duties include wound care (dressing changes, debridement), stoma-related services, catheter, or enteral tube placement and/or exchange, drain management, etc.

 

The Colorectal APP shall provide services as assigned by the supervisor in furtherance of the university's missions and goals of teaching, research, patient care, outreach, and public service. The Colorectal APP is expected to comply with the scope of practice, other laws, regulations, OHSU policy, and other applicable professional standards of practice.

 

The Colorectal APP delivers healthcare services to individuals within the scope of their license, provides advanced education to students, peers, and others; and may contribute to new knowledge and/or technology by conducting or participating in scholarship. Involvement in National/Local Colorectal organizations is highly encouraged.

 

The Colorectal APP will cross-train with the inpatient Green Surgery (General Surgery – Abdominal Wall/Hernia) APP to facilitate a coverage model for vacation coverage and absences.

 

Schedule: 4 Ten-hour days per week - Mondays must be included. Occasional Saturday or holiday coverage – planned in advance. No Call


Clinical Activities (not related to teaching or research): Assure the safe, efficient, high quality and customer oriented management of patient care to identified patient populations, including but not limited to completing a comprehensive assessment of the patient’s status; developing, monitoring and revising, as appropriate, an evidence-based treatment plan that addresses diagnoses, patterns of disease, risk factors, and treatment alternatives; treat many common acute and chronic health problems; interpret diagnostic tests prescribe and manage medications and other therapies, provide health teaching with an emphasis on prevention of illness and health maintenance; and, refer patients to other healthcare professionals as necessary.

  • Knowledge and competency in basic management of postoperative Colorectal Surgery patients.
  • Educates the patient and families throughout their stay for discharge
  • Assures effective and efficient workflow and length of stay by partnering with the discharge case manager daily and at interdisciplinary care coordination rounds.
  • Outpatient care management includes, but is not limited to comprehensive physical assessment, ordering/interpreting diagnostic tests, patient/family education, follow up management, consultation with bariatric team and referral to specialty services

Scholarly Activities (improvement science, research, writing, abstracts, grant supported work, presentations, etc.): Keep abreast of changes in field and applies knowledge to identify and recommend enhancements to clinical quality, patient experience/satisfaction, length of stay and to achieve improvement that demonstrates a higher standard of care. Assesses and recommends initiatives to improve performance indicators related to patient experience, quality, and costs. Professional development meeting the expectations of ongoing certification. Demonstrates success or productivity in the field as evidenced by securing research funding, publication in journals and/or other forms of peer presentations.

  • Actively contributes to Quality Improvement clinical projects. Collaborate with residents to support research endeavors (when appropriate).

Teaching/ Clinical Supervision: Support OHSU’s academic missions by participating in the design, development and assessment of program curricula for professional specialty area; including course objectives, instructional methods, learning outcomes, projects and assessment measures. Teach program courses as needed according to specified learning outcomes; academic advising and career related counseling for students; monitoring student retention and identifying and assisting at-risk students. May serve as the faculty of record and principle contact for extern placements and student’s performance at the assigned site.

Deliver individual instruction to support fellows, residents and other clinical learners to support their personal program of learning in the applicable specialty area. Provide instruction and review of the practices, policies and procedures in a variety of functions; including but not limited to development of treatment plans, interpretation of clinical diagnostic data; developing orders; patient and family communication and education; management of medical equipment, electronic medical records; performing procedures, and transitioning patients to the next level of care. Assures adherence to the specialty specific protocols, best practices, quality, and organizational indicators. Provides APP in put to the performance of the resident as a provider in the clinical setting.

  • Creating pathways and protocols to standardize care for Colorectal Surgery patients. Updating existing pathways/protocols. Creating Order Sets to facilitate standardized care.

  • Regular and consistent attendance at work
  • Current licensure as required by specific Advanced Practice Provider governing board(s).
    • Current Basic Life Support (BLS) certification
  • Compliance with Code of Conduct, Respect in the workplace and Applicable policies, procedures and agreements related to position, department or OHSU as a whole

  • Must be able to perform the essential functions of the position with or without accommodation


  • Prefer 2+ years clinical experience as an APP

  • Experience using Epic EMR
  • APP National Certification, Oregon Nurse Practitioner license with prescriptive authority or PA license
  • Advanced Cardiac Life Support (ACLS)

  • Hours vary.
  • Frequent interruptions in an environment of frequent change and fluctuations.
  • May be exposed to excessive auditory and visual stimulation.
  • May be required to attend out of town conferences according to hospital travel and reimbursement policies.
  • May be exposed to body fluids, tissue, radiation exposure, and infectious processes.
  • Subject to computer work for several hours, as needed.
  • Able to use all appropriate patient care equipment and office equipment within the work area.
  • Standing: On concrete, vinyl, or carpeting up to 4 hours per day and intermittently.
  • Sitting: Chair or stool continuously up to 8 hours per day and intermittently.
  • Changing Position: Intermittently, as needed.
  • Walking: Intermittently up to 4 hours per day on concrete, vinyl, and carpeting.
  • Carrying: Up to 50 pounds (supplies, equipment, books, papers, etc.) for intermittent periods and distances.
  • Lifting: Up to 50 pounds (patients, equipment, supplies, books, etc.) Intermittent throughout the day.
  • Pushing/Pulling: Up to 100 pounds, intermittent, throughout the day.
  • Bending: From the waist and knees, frequently throughout the day.
  • Reaching/Handling: Fine manipulation, repetitive, gross motor requiring full ROM in upper extremities, including overhead reaching.
  • Twisting: All ROM required, intermittent periods, throughout the day.
  • Climbing: Staircase, intermittently throughout the day.
  • Crawling: Infrequent but possible on vinyl, carpet surface
