JOB TITLE: Advanced Practice Nurse - Palliative Care

JOB STATUS: Full-time, benefits, salaried/exempt.

  1. POSITION SUMMARY
According to prescribed policies and procedures of the organization including all applicable state and federal regulations and under the general supervision and in conjunction with the Director of Palliative Care and collaborative physician, the Nurse Practitioner is responsible for performing patient medical examinations and Palliative Care visits. Documents visits in the patient’s clinical record. Provides medical care services and Palliative Care intervention for Palliative Care patients. The Palliative Care Nurse Practitioner communicates effectively with the collaborative physician and the Palliative Care Interdisciplinary Team.
  1. PRIMARY RESPONSIBILITIES
The following activities shall comprise the primary responsibilities of the Nurse Practitioner:
  • Initial visits including transitional care visits where appropriate
Intervention/Patient Care Coordination
  1. Performs medical examinations and patient home visits based upon patient need.
  2. Assist in managing the patient’s symptoms including prescribing medication or care by coordinating with referring providers.
  3. Able to exercise independent judgement, critically analyze situations, and make recommendations using appropriate problem-solving skills.
  4. Able to function effectively as a member of a team.
  5. Must have excellent interpersonal skills and the ability to work and communicate within all levels of the organization.
  6. Effectively communicates in interpersonal, professional and public relationships.
  7. Participates in the Quality Assessment/Performance Improvement Program of the agency as appropriate and needed.
  1. HUMAN RELATIONS
  1. Provide the Services in timely and professional manner and in accordance with; (i) the bylaws and operational policies and procedures of Palliative Care, including without limitation, its Compliance Program; (ii) Palliative Care’s written policies and procedures including those related to billing submission; (iii) local, state and federal laws and regulations
  2. Demonstrates and maintains a positive, professional manner, and manages time efficiently and effectively.
  3. Develops and maintains effective and positive relationships with the interdisciplinary group, other professionals and physicians in community.
  4. Must have an interest in and a sympathetic, compassionate attitude toward caring for the patient.
  5. Must maintain a professional attitude and demeanor and is able to communicate effectively and relate courteously and cooperatively with patients, caregivers, colleagues, supervisors, co-workers and all others.
  6. Must be able to demonstrate optimism, enthusiasm and willingness to work constructively with other healthcare team members and reflect concern for the well-being of patients and employees.
  7. Must interact with all staff in a positive fashion supporting the mission and vision of the organization.
  1. PHYSICAL REQUIREMENTS
  1. Visual/hearing ability must be sufficient to comprehend written/verbal communication.
  2. Must have the ability to speak, read, write and understand English.
  3. Must have excellent verbal and written communication and interpersonal skills.
  4. Able to effectively manage stress and demonstrate time management skills.
  5. Must be able to meet the physical demands of the position.
  6. Able to work in a variety of settings and conditions in patient’s homes and facilities.
  1. EDUCATION AND EXPERIENCE
  1. Education:
  1. Master’s Degree in nursing from a school accredited by a nursing accrediting association recognized by the U.S. Department of Education
OR
  1. Master’s Degree in nursing and completed a post-master’s program that focuses on an Advanced Practice nursing specialty from a school accredited by a nursing accrediting association recognized by the U.S. Department of Education.
  1. Experience:
  1. Minimum of two (2) years’ experience as a Nurse Practitioner is preferred.
  2. Must have an understanding of Palliative Care.
  1. Licensure, Registration, and/or Certification
  1. Holds a current, valid Registered Nurse license.
  2. Currently certified as a Nurse Practitioner by a national certifying agency that is accredited by the American Board of Nursing Specialties and/or the National Commission for Certifying Agencies per the State Board of Nursing.
  3. Valid driver’s license and reliable transportation that is insured in accordance with State requirements.
  4. Satisfactory criminal, motor vehicle background checks, professional references and drug screening.
