JOB TITLE: Advanced Practice Nurse - Palliative Care
JOB STATUS: Full-time, benefits, salaried/exempt.
- POSITION SUMMARY
- PRIMARY RESPONSIBILITIES
- Initial visits including transitional care visits where appropriate
- Performs medical examinations and patient home visits based upon patient need.
- Assist in managing the patient’s symptoms including prescribing medication or care by coordinating with referring providers.
- Able to exercise independent judgement, critically analyze situations, and make recommendations using appropriate problem-solving skills.
- Able to function effectively as a member of a team.
- Must have excellent interpersonal skills and the ability to work and communicate within all levels of the organization.
- Effectively communicates in interpersonal, professional and public relationships.
- Participates in the Quality Assessment/Performance Improvement Program of the agency as appropriate and needed.
- HUMAN RELATIONS
- Provide the Services in timely and professional manner and in accordance with; (i) the bylaws and operational policies and procedures of Palliative Care, including without limitation, its Compliance Program; (ii) Palliative Care’s written policies and procedures including those related to billing submission; (iii) local, state and federal laws and regulations
- Demonstrates and maintains a positive, professional manner, and manages time efficiently and effectively.
- Develops and maintains effective and positive relationships with the interdisciplinary group, other professionals and physicians in community.

- PHYSICAL REQUIREMENTS
- Visual/hearing ability must be sufficient to comprehend written/verbal communication.
- Must have the ability to speak, read, write and understand English.

- Able to effectively manage stress and demonstrate time management skills.
- Must be able to meet the physical demands of the position.
- Able to work in a variety of settings and conditions in patient’s homes and facilities.
- EDUCATION AND EXPERIENCE
- Education:
- Master’s Degree in nursing from a school accredited by a nursing accrediting association recognized by the U.S. Department of Education
- Master’s Degree in nursing and completed a post-master’s program that focuses on an Advanced Practice nursing specialty from a school accredited by a nursing accrediting association recognized by the U.S. Department of Education.
- Experience:
- Minimum of two (2) years’ experience as a Nurse Practitioner is preferred.
- Must have an understanding of Palliative Care.
- Licensure, Registration, and/or Certification
- Holds a current, valid Registered Nurse license.
- Currently certified as a Nurse Practitioner by a national certifying agency that is accredited by the American Board of Nursing Specialties and/or the National Commission for Certifying Agencies per the State Board of Nursing.
- Valid driver’s license and reliable transportation that is insured in accordance with State requirements.
- Satisfactory criminal, motor vehicle background checks, professional references and drug screening.
