HIV Specialty Care- Advanced Practice Clinician (FNP or PA)

Western North Carolina Community Health Services (WNCCHS) is a Federally Qualified Health Center (FQHC). We are looking for an Advanced Practice Clinician (APC) – Nurse Practitioner (FNP) or Physician Assistant (PA) – to provide professional medical services to complement the delivery of integrated team-based, whole-person, trauma-informed primary health care services through a Patient Centered Medical Home (PCMH) model of care.
The role of the APC is to assess, maintain and improve the health status of individuals in the communities served through timely access to evidence-based acute, preventive, and chronic disease care. The APC effectively collaborates with and/or delegates to members of the interdisciplinary health care team and provides indirect supervision of medical support staff.
Primary focus: Provides ongoing management to adults living with HIV including diagnosing acute HIV infection; initiation and modification of appropriate antiretroviral therapy; prevention and treatment of acute opportunistic infections; monitoring clinical and immunological status; routine preventative health screenings unique to HIV patients.
KEY RESPONSIBILITIES- Includes but is not limited to the following:
HIV Specialty Care Focus
Under the clinical supervision of a collaborating physician, APC provides comprehensive medical care to a panel of patients primarily comprised of adults living with HIV. This includes screening patients for HIV, offering prevention tools to those who screen negative, diagnosing acute HIV infection; initiation and modification of appropriate antiretroviral therapy; prevention and treatment of acute opportunistic infections; monitoring clinical and immunological status; and routine preventative health screenings unique to HIV patients.
Advanced Practice Clinician
  1. Engages in a patient-centered approach to provide timely access to high quality care for patients and their families.
  2. Serves as a role model of professionalism to all members of the interdisciplinary health care team. Collaborates with all Licensed Independent Practitioners (LIP) and Other Licensed Independent Practitioners (OLIP) including Physicians, APCs, Therapists, Dentists, etc., and clinical support staff to form a high performing team.
  3. Cultivates a climate of safety, trust, and compassion for patients and colleagues.
  4. Provides medical evaluation, treatment, and consultation services to patients of the clinic in a timely manner, including responding to emergencies and answering medical questions.
  5. Provides high-value cost-conscious care; demonstrates good stewardship of resources by limiting unnecessary testing and referrals; and considers risks, benefits, and patient preferences when making diagnostic or therapeutic intervention decisions.
  6. Documents all patient care activities and bills for all services provided in the electronic health record. Documentation must be clear, concise, accurate, updated, and in sufficient detail to allow another provider to continue the patient’s care as well as to allow other parties to review the safety and quality of care provided.
  7. Obtains and documents appropriate chief complaint, history of present illness, immunizations, medications, allergies, medical, surgical, family, and social histories, review of systems, examination, preventive services, and assessment and plan for each patient.
  8. Provides medical orders for prescription medications and refills, administration of medications, preventive care, ancillary testing, and/or referrals as needed to patients.
  9. Reviews and analyzes reports, test results, medical records, and examinations in a timely manner to diagnose condition of patients, propose treatment options and coordinate notification and follow-up to patients.
  10. Provides culturally and linguistically appropriate education to patients and/or families as to the disease's nature, provides instruction on proper care, self-management, and treatment, and works with patients to develop individualized care plans.
  11. Continually monitors implementation of care plan.
  12. Performs medical/surgical procedures within scope of practice/training/skills.
  13. Coordinates transitions of care with other settings/providers (e.g., inpatient hospitals, emergency departments) and supervises patient care by non-clinic agencies (e.g., home health, nursing home, and hospice care).
  14. Remains available to patients personally or through clinical staff to answer questions and relay information regarding their care.
  15. Completes all appropriate paperwork in a timely manner, including documentation to comply with insurance and reimbursement guidelines.
  16. Trains and supervises health professions students and allows for the shadowing of medical residents as needed or directed.
  17. Actively participates in quality improvement activities including initiating or contributing to monitoring, measuring, analyzing, improving and/or controlling program goals, objectives and/or services.
  18. Reviews peer PCP (Primary Care Provider) charts when appropriate, gives feedback to peers to support the delivery of high quality, evidence-based care, and promotes a culture of peer education and continual learning.
  19. Complies with current incentive, regulatory and certification reporting requirements (such as Managed Care, ACO, MIPS, PCMH, and UDS) as directed.
  20. Participates in interdisciplinary care/treatment teams and directs the patient care activities of nursing and support staff as required.
  21. On a rotating basis, provides medical assessment and advice to the on-call nurse and/or to patients, their families/caregivers by telephone when the clinic is closed.
  22. Remains up to date with best practices, developments, treatments, and medications.
  23. Adheres to policies and protocols, quality improvement objectives, and safety, environmental and infection control standards.
  24. Maintains patient confidentiality and complies with all federal and state health information privacy laws.
  25. Performs other duties as assigned by Chief Medical Officer and/or members of the WNCCHS leadership team.
QUALIFICATIONS:

Education: Graduation from an accredited master’s degree program as a Nurse Practitioner or Physician Assistant
Certification/Licensure:
  • Current North Carolina licensure/certification required (FNP or PA)
  • Current DEA license
  • BLS (Basic Life Support) certificate must be current or obtained within 30 days of employment
  • Must be Medicaid and Medicare eligible
  • American Academy of HIV Specialist (AAHIVS) certification is strongly preferred
Experience:
  • Experience with pre-exposure prophylaxis, post-exposure prophylaxis, HIV treatment, and transgender care
  • Excellent verbal and written communication skills
  • Knowledgeable about current standards, methods, and procedures for the delivery of evidence-based care in an efficient, organized, and accurate manner
  • Collaboration and teamwork skills to work efficiently with others to provide innovative ideas and help inspire staff and management
  • Time management skills commensurate to a high-volume patient panel
  • Interpersonal skills, compassion, and empathy to build positive relationships with patients and colleagues. Flexibility to work with a patient population diverse in age, personality, and background
  • Previous outpatient or FQHC experience is preferred
  • Bilingual in English and Spanish and/or Russian is preferred
  • Experience in public health is preferred
