JOB SUMMARY
Responsible for providing integrated comprehensive medical and psychiatric care for New Age Services Corporation patients, assisting patients to attain to and maintain optimal wellness in the community.
JOB DUTIES
· Assume primary responsibility to address the health and wellness needs of each client, identified and/or extracted from individual Treatment/Service Plans.
· Develop strategies to promote Health and Wellness models including interface with Federally Qualified Health Center, collaborative agreements with specialized medical facilities, partnerships with local hospitals or centers of excellence.
· Collection of key health indicators, such as percentages of Clients with complex co-morbid medical conditions, including, cardiology, COPD, brittle diabetes, hypertension, obesity, etc.
· Use of standardized collection measures; contribute to the development of an effective quality assurance and utilization review functions.
· Observes all Payer standards and procedures in the program including those for Medicaid and Medicare treatments, physical examination, medical referrals, and medication of clients; provide orders/prescriptions for medications.
· Assist in implementation of a total quality management approach in medical and clinical operations.
· Participate on committees and teams designed to evaluate and improve the efficiency, effectiveness, and quality of services to clients.
· Provide medication monitoring training and authorization for those qualified to provide medication monitoring.
· Assist with securing hospitalization, when needed.
· Provide consultation to staff around clinical issues as they relate to treatment of clients.
· Participate in multi-disciplinary staffing and clinical team meetings.
· Implements processes that will allow for efficiency in clinic.
· Document all activities on required documents.
· Appropriate time management while performing duties at NASC facilities.
· Receptive to and respectful of, supervision by Medical Director, Primary Care Provider and Management Team
· Other duties as assigned.

SPECIFIC DUTIES
· 100% of Clients will have a physical evaluation within 30 days of referral.
· 100% of Clients will have an annual physical evaluation for the duration of the enrollment in NASC services.
· 100% of Clients with abnormal blood sugar will be referred to a primary health care physician.
· 100% of Clients diagnosed with diabetes will be trained to administer medications.
· Clients with frequent ER visits within three months will have a multi-disciplinary consultation with the primary care physician.
· 100% of Critical Incidents due to deteriorating health will be staffed for possible increase in level of care.
· Field (out of office) maybe required for linkage agreements.
Job Type: Full-time
Pay: $120,000.00 - $135,200.00 per year
