HOW WILL YOU HELP OTHERS AT UCP HEARTLAND?
--You will be Involved in the Individualized Service Plans (ISP) process. Implement and monitor outcome objectives as specified in the consumer’s ISP.
--You will schedule and implement daily activities including those to maintain or improve independent living skills, meal planning and preparation, recreational and leisure, social, physical wellness, service to the community and hygiene.
--Provide direct support and care to the clients including accessing the community (we love to take excursions and so do our clients--you can make their whole day!)
--Other work as assigned.

DIMENSIONS
--UCP Heartland DSPs report to the assigned Training Supervisor for programmatic supervision and guidance of the day-to-day operation of the program. The primary focus is on assisting enrolled consumers in the achievement of greater independence.
--Flexibility is expected, since the DSP may be required to substitute in other program areas and/or locations when needed. Persons trained, approved, and assigned to use Agency vehicles must ensure that vehicles are operated in a safe and effective manner and must complete an agency-sponsored driver training program, and maintain a clean driving record.
--Maintain cooperative professional working relationships with all Agency consumers, employees, guardians, and families. Must consistently display appropriate conduct as a role model for consumers. Interactions with families, primary caretakers and others may be limited, but professionalism is always expected. The DSP is a member of the consumer’s support team. In speaking with any person, confidentiality must always be maintained.
--Exercise creativity to ensure that consumers are engaged in meaningful activities that support their outcome objectives. As employee-consumer ratios fluctuate between 1:1 and 1:6, appropriate engagement tasks and activities geared towards each person’s unique needs and expectations are required on a constant and consistent basis. ----There will be times in which all outcome objectives have been addressed, and the DSP must consistently have alternative age-appropriate meaningful activities prepared. It is further expected that the DSP will actively work towards improving the quality of services offered.

ESSENTIAL JOB FUNCTIONS
• Actively participate as a member of a team in goal development for consumer support plans.
• Plan, prepare for, and implement individual and small group daily habilitative activities that are meaningful, age-appropriate, and support each assigned consumer’s program plan.
• Provide center-based or community-based services in accordance with the person-centered plan.
• Ensure that all consumers are actively engaged and supervised, and that no consumer is left unsupervised at any time.
• Monitor and collect supporting data for consumer outcomes and measures.
• Complete all required reporting accurately, in a timely manner, and in accordance with guidelines established by the Agency and other Regulatory Entities including but not limited to service documentation, incident reports, and progress notes.
• Support consumers with meals, toileting, dressing, bathing, and other personal care.
• Remain in compliance with all program's regulatory codes and requirements (i.e., Medicaid, DMH, SB40 Boards).
• Ensure that CARF standards, Agency, and departmental policies and procedures are consistently followed.
• Ensure that all assigned outcomes, data collection, and progress reporting are completed accurately, thoroughly, and within Agency time frames.
• Maintain training and certifications consistent with Agency policy and procedure.
• Ensure the Health and Safety of program participants and staff.
• Maintain HIPAA compliance and confidentiality.
• Perform heavy physical work as needed to assist in the care of a consumer. Heavy work involves exerting force to lift, carry, push, pull, and otherwise move individuals and objects who may weigh in excess of 150 pounds and, involves bending, squatting, reaching, and moving about most of the time.
• Use proper transfer techniques and/or mechanical lifts.
• Safeguard and assure proper care and maintenance of consumer and Agency property including but not limited to adaptive equipment, lifts, and Agency vehicles.
• Effectively communicate orally and in writing.
• Promptly communicate any problems or concerns that may arise.
• Comprehension and proficiency with all agency software used for programs
• Secure and maintain training/certification in CPR/First Aid, CPI, abuse and neglect, bloodborne pathogens, HIPAA, and all other certifications or trainings as required by the Agency.
• Use the EHR (Electronic Health Records) system to complete all applicable documentation
MINIMUM EMPLOYMENT STANDARDS
High School diploma or its equivalent (GED).
Experience supporting individuals with disabilities.
Ability to take direction and work with a minimum of supervision.
Ability to consistently work in a professional, cooperative manner with all Agency consumers and personnel.
Good written and oral communication skills.
Must have a valid driver license with a good driving record.
Computer proficiency

PREFERRED EMPLOYMENT STANDARDS
Associate degree in a human service field.
Two years’ experience supporting individuals with developmental and physical disabilities.
Knowledge of and experience with adaptive equipment and assistive technology devices for adults with developmental and orthopedic disabilities
Knowledge of and experience with Electronic Health Records system
