Overview
ESSENTIAL FUNCTIONS:

  • Responds to all new initial and follow up contacts for admissions to Genesee Lake School residential, day school, and respite programs by phone, email, fax and social media.
  • Gathers, records, and assesses all information necessary to assure the placement meets the therapeutic needs of the individual, and enters information into client databases, and uploads referral documents into Sharepoint. Reviews referral documentation (including IEP and other academic records, psychological evaluations, medical records, and any other pertinent documents) for accuracy and completion.
  • Acts as a liaison between the facility, Admissions team, family members, referral sources, and other stakeholders during the pre-admission and admission process.
  • Attends weekly Admissions Meeting. Prepares a summary document of data collected, presents information, and makes a recommendation for admission. Monitors transfers, discharges and the admissions wait list. Reviews each student's information with the team in regards to education, health, and social needs. Gathers additional information for programming, therapeutic, and education staff, and follows up with all requested information. Schedules and facilitates tours of each GLS service offering and provides information appropriate to the requested service offering. This may include any or all information regarding residential, educational, vocational, and respite offerings.
  • Conducts a visit to interview and observe the student, either by an on-site visit or by using technology, such as Zoom, Skype, and Facetime to gather additional information to compare observations to referral information received. Conducts information seeking conversations with all appropriate contacts, including but not limited to: school districts, parents and family members, referring agencies, mental health professionals, and any other relevant stakeholder in students' life.
  • Assesses the viability of funding sources by ensuring payment mechanisms are identified and workable prior to admission.
  • Communicates outcome of admission decision to parent/guardian and referral source. If admission is approved, sets the admission date, completes New Student Checklist assuring all enrollment documents are received and completed, and prepares packets once admission is approved.
  • Assists with student de-escalation and redirection as inserviced by the company.
  • Conducts self in positive, respectful and collaborative manner demonstrating proactive actions and decision making that foster supportive work relationships and a healthy and safe environment. Communicates effectively verbally and in writing with internal and external customers maintaining professional conduct and confidentiality.
  • Maintains abreast of current trends in Children's Services and other related areas; attends conferences, workshops and other training as needed and shares this new information with department.
  • Complies with and positively reinforces with others the expectations outlined in: Company Handbook; policies and procedures including emergency/disaster procedures, fire/tornado safety and maintains HIPPA confidentiality.

KNOWLEDGE, SKILLS, AND ABILITIES:
• Bachelor's Degree in Education or Human Services required.
• Two years previous work experience in a human services field or education required.
  • Proficient in Word and Excel required. Previous experience working in databases preferred.
  • Excellent critical thinking, assessment skills, and problem-solving skills; verbal and written communication skills, and strong customer service skills required.
