Admissions RN - Hospice & Palliative Care

Hospice Alliance is seeking a compassionate RN to join our admissions team. In this role, you will conduct consultation visits to assess the patient's eligibility for hospice and Palliative Care services and Hospice & Palliative Care admissions.

Prior Palliative Care, home health and/or hospice experience and CHPN certification is highly desirable. Communication, critical thinking , and time management skills are essential in this position.

This position works Monday - Friday, 10:30am to 7pm. Participation in a weekend and holiday rotation is also required.

Travel to the patients place of residence to complete the admission or consult if required. Travel is primarily within Kenosha and Racine counties and some portions of Walworth.
