Qualifications
• BLS certificate
• Excellent organizational skills to handle a variety of tasks simultaneously
• Ability to accommodate multi-faith beliefs and backgrounds of patients/families
• Ability to function as part of interdisciplinary team
• Must have reliable, insured vehicle and valid Texas Driver’s license
• Bachelor's (preferred)/ADN with 4yrs of experience.
• Current CPR certification (Required)
• Current RN license to practice in the state of Texas (Required)
Responsibilities
• Communicate with staff, physicians, referral sources and prospective clients regarding hospice services and appropriate admission criteria
• Handle initial physical and emotional assessment of the patient/family upon admission and reviews the hospice concept with them
• Develop a detailed, written POC for each patient admitted in conjunction with the IDG, patient, family and attending physician
• Explain to patient and family what items hospice pays including a review of the medications list
• Instruct patient/family about appropriate techniques for care of the patient
• Communicate with PCM, patient DME, HHS and medication needs
• Communicate to the IDT report on new admissions and prioritizes needs
• Participate in orientation of staff and continued education of medical students, nursing students, and other health care professionals including community as appropriate
• Assist with routine visits if no admissions are scheduled for the day
• Perform other job related duties as required and/or requested by management
