POSITION SUMMARY
The purpose of your job position is to obtain the required information and admit residents in an efficient manner, with the facility's established policies and procedures, and as directed by management.
The Admissions and Social Services Assistant will treat each resident fairly, kindly, and show them dignity and respect. They will refer to each resident by their name. They will communicate clearly, empathetically, and effectively when speaking to residents, family members, visitors, all staff and governmental agencies. They will work cooperatively with all departments and multidisciplinary teams. They will demonstrate patience, initiative and willingness to assist residents that may be difficult or in discomfort. They will relate all pertinent information concerning a resident's condition to a charge nurse as required. They will be committed to always doing the right thing.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Keeping a log of all admission packet sign-ins
Meeting with newly admitted residents and ensure 100% compliance in admission packet sign-ins
Partnering with Administrator, Marketing Director and Social Services to complete tasks
Conducting sales calls to physicians, hospitals, medical groups, etc. in conjunction with the Marketing Director
Updating the Customer Relationship Management (CRM) module in Point Click Care daily
Overseeing inquiries from physicians, hospital discharge planners, etc. and Respond to all referrals within 15 minutes of receipt
For PDPM compliance, ensure all new admissions to the facility have proper documentation discharge summary, therapy notes, nursing notes, and medication list
Protecting each residents file and data with confidentially per HIPAA standards
Making phone calls to set up transportation or doctor appointments
Providing residence and family Medicare/Medicaid and financial information when requested
Maintaining social service progress notes according to the resident’s treatment plan
Updating the social service directory
Obtaining information from community social health and welfare agencies to meet residents needs
Protecting each resident's file and data with confidentiality per HIPAA standards.
Abiding with all facility policies and procedures including not disclosing user ID codes and passwords
Providing tours in a professional manner
Attending meetings as required

REQUIREMENTS
Education / Licensure
Must have a high school diploma or GED
Social Services Designee (preferred)
Qualifications / Experience
Must have minimum of 1 year experience working admissions and/or social services in skilled nursing setting
Must be able to read, write, understand and communicate proficiently in English language
Must be familiar with Medicare, HMO, Medi-Cal and medical terminology

May be requested to work beyond scheduled working hours at times
May be exposed to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses
Physical Requirements
Must be able to move intermittently throughout the day
Must be able to push, pull, move and or lift up to 25 lbs. to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 15 feet
