  1. Plan, implement and coordinate all admissions for 2-10pm shift, including nursing services for SNF residents.
  2. Assist In-service Director in coordination of orientation of new trainees and

in-service on nursing department employees.

  1. Assist DON in assuring quality nursing care and agency compliance with

Medicaid, Medicare and State Health requirements.

  1. Responsible for promoting and maintaining good interpersonal and

interdepartmental relationships and carrying out administrative procedures in conformity with BRNC policy.

  1. Responsible for enhancing professional development of staff in areas of creativity,

skills behavior and personal appearance.

  1. Responsible for keeping DON, Nurse Coordinator and/or Resident Care

Coordinator fully informed regarding needs, unusual occurrences, problems, and accomplishments in order to establish and maintain lines of communication and to enhance continuity of quality resident care.

  1. Performs duties of staff nurse as required in emergencies and whenever

necessary at BRNC and TLC.

 

SPECIFIC DUTIES:

1.         A.        Plan effective admissions/administration of 2pm-10pm shift.

  1. Coordinate through DON, Resident Care Coordinator, other

Supervisors and Staff Nurses all unit management and resident care tasks.

  1. Responsible for knowing Fire, Safety and Disaster Manuals for

BRNC and TLC and how to direct subordinates in prevention drills, emergencies and other related situations.

  1. Responsible for the care and use of supplies and equipment.

 

 

  1. Assess and evaluates nursing service needs.
  1. Completion of specific required supervisory assessments (Falls,

Incontinence, Pressure, etc.)  as well as responsible for obtaining required documentation for resident/family regarding Preferred Intensity of Care, Advance Directives, No CPR, and use of siderails, within one week of admission, as requested by DON or Resident Care Coordinator.

  1. Assist MDS Coordinator with identification of residents’ problems

and care plan initiation and implementation, including periodic resident reassessment, care plan evaluation and revision within regulatory-set time frame.

  1. Assist Resident Care Coordinator with weekly Utilization Review

Assessment of Skilled Residents and weekly SNF Summary on Skilled Residents as requested.

II.        A.        Assist DON in evaluation nursing personnel as directed by policy or as

                        indicated.

III.       A.        Assist, as delegated by Inservice Director, orientation of staff nurses to the

                        unit.

  1. Assist, as directed by Inservice Director, orientation of new licensed staff

to medication system.

  1. Assist staff nurses in assignment of GNAT’s to work with CNA’s.
  2. Assist staff nurses with GNAT Unit Performance Evaluations,
  3. and communicates to DON/GNAT Instructor any additional educational needs of the GNAT.
  4. Assist staff nurse in scheduling CNA’s for continuous inservice education with Inservice Director.
  5. Promote favorable attitude regarding orientation and inservice education.

IV.       A.        Assure required monthly summaries be written with substance and in

                        compliance with Medicaid, Medicare and State Health documentation

                        requirements.

  1. Assure physician’s orders are properly transcribed and implemented.
  2. Responsible for assigned resident care plans.
  3. Assist 7-3 Supervisor with evaluation and documentation of ongoing Restraint Reduction Program and for tracking of siderail usage for QAA Committee.
  4. Responsible for obtaining physician’s orders for nursing procedures requiring an order.
  5. Assure narcotics and controlled drugs are in correct count for 7am-7pm shift.
  6. Assure medications are given as directed or responsible withheld, with appropriate documentation.
  7. Assist DON, MDS Coordinator, and Resident Care Coordinator in review of residents’ charts to assure medical and nursing compliance with Medicaid, Medicare, and State Health requirements.
  8. Responsible for supervision of assigned CNA/Staff Nurse for Skilled Unit and of skilled resident care, ensuring proper documentation and quality care.
  9. Participate in Nursing Environmental Rounds and audits for QAA.

VI.       A.        Promote harmonious relationships among co-workers and other

                        departments, including the administration, as well as with residents,

                        families and visitors.

  1. Responsible for following and fostering use of established lines of

communication within the nursing department and between departments.

  1. Assures personnel conformity to BRNC policy.
  2. Serve as a channel to administration in conveying staff’s suggestions for effective change.

VII.      A.        Assume responsibility for own professional growth and development

                        through participation in Inservice programs, attending staff meetings and

                        reading professional journals, etc.

  1. Interpret principles of good management to nursing personnel and serve as

a role model in implementation and application of these principles in daily work.

  1. Provide opportunities for staff to solve problems and to develop

management and nursing skills under appropriate supervision.

  1. Recognize individual and group strengths, and utilizes strengths to

to improve weaknesses.

  1. Any other duties requested by Administration or DON.

 

JOB QUALIFICATIONS

  1. Graduate of an accredited school of nursing.  Current registration with the

Washington State Board of Nursing.

  1. At least one year of experience in supervisory capacity in a hospital or extended

care facility, including experience in geriatric nursing.

  1. Good mental and physical health, sound judgment and high moral standards, and

a sincere desire to work with the aged and those with limited capacity for self-care.

  1. Ability to manage and supervise a nursing unit.  Must show interest and ability

when teaching others and assisting them in their own skills.

  1. Capacity to grasp new theories and principles, and to put them into effective use.
  2. Ability to make independent decisions.
  3. Verbal ability necessary to understand resident charts, doctor’s orders, nursing care plan and medication orders, and to communicate with residents and staff.
  4. Manual dexterity necessary to coordinate hands, eyes and fingers in administering medications and treatments.  Some clerical perception is required to prepare records and charts.  Knowledge of computers is useful.
  5. Attendance at workshops and institutes in this field is encouraged.

ESSENTIAL JOB FUNCTIONS
  1. Admitting patients between 2pm-10pm
  2. Perform duties of a staff nurse as needed.
  3. Assist 6-2 Supervisor with Physician Rounds when necessary, and see that scheduled Recertifications, orders, and progress notes are signed and dated correctly by the physician, and that all orders are noted and transcribed correctly by the nurse.
  4. Supervise SNF resident care and associated documentation on 2-10 shift.
  5. Assist 6-2 Supervisor with evaluation and documentation of ongoing Restraint Reduction Program and siderail usage.
  6. Participate in weekly Environmental Rounds and audits for QAA.
  7. Supervise taking of call-ins for absences, late arrivals, etc., during 2-10 shift, and assure reasonable attempts for replacement or schedule adjustments for adequate coverage.
  8. Assure timely response to TLC requests for licensed nursing intervention during 2-10 shift.
  9. Assure appropriate nursing assessments and interventions, physician notifications, family notifications, and administrative notifications for all significant occurrences on 3-11 shift at BRNC and TLC.
  10. Assist MDS Coordinator with resident assessments and problem identification for Care Planning; participates in Care Planning when possible.
  11. Participate in QAA Committee and Subcommittee Meetings as directed.
  12. Meet and greet families and residents on assigned new admissions, and assure required documentation regarding assessments, orders, No CPR, Advanced Directives, and Preferred Intensity of Care, and Restraint.
  13. Assist Resident Care Coordinator with weekly SNF Medical Summary completion and Utilization Review Assessments.
  14. Assist 7-3 Supervisor with ongoing evaluation of pressure ulcer treatments and interventions and assure completion of associated documentation.
  15. Plan, implement and coordinate all nursing services for 7pm – 7am shift, including nursing services for SNF residents.
  16. Supervise and evaluate the performance of staff nurses, CNA’s, NA Helpers and
  17. Unit Secretaries on 7pm-7am shift.
