Duties and responsibilities:

• Coordinate patient care
• Build and maintain relationships with all members of the patient’s care team, both inside and outside Lucent Home Health, to help ensure continuity of care
• Advise Intake Department and management on new, prospective, and existing patient needs
• Coordinate with the Intake Department to ensure all necessary documentation is received and care commenced
• Serve as a point of contact for the patient for any potential issues, including but not limited to changes in underlying health conditions
• Update the Intake Department on any pending changes in patient status
• Maintain precise, accurate, and confidential records of patient information

Qualifications

• At least two years of experience in a healthcare related business development position
• Batchelor’s degree preferred
• Driver’s license, liability insurance and reliable transportation
• Able to work autonomously with self-motivation
• Microsoft Outlook experience a must
