Job Summary

To provide support to young adult students with Developmental Disabilities, Intellectual Disabilities or Autism who are enrolled in a post-secondary education program. The Transition Academy Support Specialist will provide support in all Program areas of the Transition Academy, which will include but not limited to providing daily transportation for students to and from school, work based learning, and recreational opportunities.


Essential Functions

1. Develops and maintains a sound working relationship with academic staff, resident life staff and workforce development staff.

2. Provides individuals with transportation to and from work, school, appointments, recreational opportunities, etc.

3. Work flexible schedule based on needs of students served and needs of the A New Leaf Transition Academy.

4. Maintain individual's confidentiality, safeguards individual's rights.

5. Interacts with students served to promote self-esteem, confidence, and morale.

6. Participates actively to ensure the wellbeing of students.

7. Treat students with dignity and respect.

8. Fulfills all training, in-service, and background check requirements necessary to work with students served.

9. Maintain professional competence in the field which may include attending and participating in professional organizations, workshops, and conferences, reading professional journals and other related publications.

10. Ensures the safety and well-being of individuals served.


RESPONSIBIILTIES

  1. Implements programs developed by the individuals’ teams, to meet the goals and objectives of each student’s Personal Career Development Plan (PCDP).
  2. Completes all required documentation and activities not limited to incidents, programmatic data, daily activities, and outcomes.
  3. Encourages and assists in the planning of social and recreational activities that are of interest to the individuals being served.
  4. Participate in the Transition Academy’s substitute program as needed, which includes being cross trained and ready to fulfill the position needs of substitute instructor, substitute residential life staff, substitute student activity staff, and substitute work-based learning staff.
  5. Performs duties as requested to meet the needs of A New Leaf Transition Academy.
  6. Assist with Work Based Learning Activities, Applied Academic Activities, and Resident and Student Life Activities as needed.

Requirements:

Qualifications

  • Must be at least 18 years of age.
  • Must emulate the values of A New Leaf, Inc., which includes being client-focused, customer focused, professional, and innovative.
  • Must be able to drive, have and maintain a valid Oklahoma driver’s license.
  • Must always have dependable vehicle.
  • Must always have and maintain automobile insurance coverage.
  • Must maintain eligibility to operate agency vehicles.
  • Physical capacities must include bending, stooping, lifting a minimum of 40 pounds, as well as the ability to lift, transfer, and position individuals served.
  • Must be able to work with frequent interruptions, deal with diverse personalities during difficult/sometimes stressful situations and still complete job assignments.
  • Must be able to communicate orally, both over the phone and face-to-face.
  • Must be able to prepare and read written documentation as well as perform and understand basic mathematical functions.
  • Work flexible schedule based on schedules of persons served by and needs of A New Leaf, Inc.
  • Must follow policy and procedures of A New Leaf, Inc.
  • Ensures all individuals served are always safe in their environment and program participation.
  • Must fulfill all training and in-service requirements necessary to work with individuals served within the timeframe required by A New Leaf, Inc., and the State of Oklahoma.

Special Requirements:

  • Maintain CPR/First Aid certification.
  • Criminal arrest check will be done through the OSBI; Per Section 1025.1 et seq. Of Title 56 of the Oklahoma statues, House Bill 1790, any prospective employee with a felony or misdemeanor conviction, guilty plea, or plea of nolo contendere will not be eligible for employment. This prohibition may be disregarded if an appropriate waiver is obtained from the Office of the Director, DDSD, Department of Human Services, or his/her designees.
  • Community Services Registry checks (House Bill 1790) will be done through DDSD, Department of Human Services. The person whose name appears on the registry cannot work for A New Leaf.
  • Employees and A New Leaf are engaged in an at-will employment relationship, meaning that either is free to terminate the relationship at any time with or without reason and with or without notice.
