General Function:
Works in an interdisciplinary team to facilitate and coordinate students' continuum of care for mental health, educational support, family support services, and substance abuse prevention. Coordinates daily activities, ensuring access and utilization of clinical, community, and educational resources, and provides referral sources for students and families. Reports and communicates the needs of students to the clinical team and program directors. Facilitates and prepares students' life and educational transitions, general wellness and prevention services, and community resources. The position is funded by a grant for one year; therefore, no promise for employment beyond the period funded by the grant is promised or implied.
Education/Training/Experience:
(Basic minimum requirements are subject to change from time to time to meet the needs of the District.)

Minimum: A bachelor's degree or two years of experience in social work. A valid NM driver's license and clean driving record. Current CPR/First Aid Certification (or willingness to become certified). Knowledge and experience in special education.
Preferred: Education work experience. Mental or Behavioral health experience.

  • Familiarity with community resources, including mental health, social, financial, educational, housing, healthcare, employment, childcare, and other support that may be required.
  • Ability to advocate, network, and conduct outreach to various entities outside of the school environment to provide resources for students and parents

  • Ability to know educational foundations, goals, developmental needs, IEP plans, tracking tools, and resources to aid in students' educational plans

Essential Job Functions:
Performs all functions in accordance with established policies, procedures, safety and environmental regulations, facility and computer security policies and procedures. Functions are subject to change from time to time to meet the needs of the organization. Current essential responsibilities include, but are not limited to, the following:
  1. Will identify and reduce unmet needs within the Indigenous population; this includes working to discover the needs of the participants and their families.
  2. Will work to address health inequalities in the identified population.
  3. Will work to improve the health of program participants, which includes connecting participants to resources in the community such as medical, dental, and food resources.
  4. Will act as a care coordinator whose role it is to connect participants to resources and follow their progress toward improved health outcomes.
  5. Will work to create stronger service coordination between agencies and improved communication among community providers and stakeholders.
  6. Will assist in coordinating and connecting the participants and their families to community resources and working with them to create step-by-step plans to solve problems and meet personal goals.
  7. Will have to check, track, and plan with participants frequently and take them step-by-step through different agency applications and processes, and then report progress to Project Aware team and NASS.
  8. Will advocate for students so that they are empowered to advocate for themselves in the future.
  9. Will work within a team atmosphere with Project Aware/NASS team, providing consistent and constant updates, communicating participant's progress, needs, and obstacles, aiding in scheduling with treatment team, attending community events, and meeting with agencies that provide community support, and network to find additional resources for participants and their family.
  10. Prepare presentations /reports related to program information and progress.
  11. Organize outreach activities as needed.
  12. Other duties as assigned

Salary Range and Benefits:
$ 54,000-$55,875 Depending on years of experience.
Medical, dental, and vision benefits are available to full and part-time employees who work a minimum of 20 hours per week.

Resources Used in Performing Job Include (but are not limited to):
Standard office equipment and software platforms such as Microsoft Office (Word, Excel, PowerPoint) and the Google Suite. Telephone, fax, copier, computer, calculator.
Physical/Cognitive Requirements:
Regularly required to stand and move throughout a room and/or facility. Communicates verbally and electronically. Uses hands to handle or feel objects, tools, or controls and reach with hands and arms. Frequently required to stoop, kneel, crouch, and/or crawl. Occasionally required to sit, climb, balance, and drive. May have occasion to perform heavy lifting up to 25 pounds. Specific vision abilities required include close vision, distance vision, color vision, and the ability to adjust focus.Must possess and maintain a valid driver's license and private automobile. Some evening or weekend work and travel may be required

Proficient use of standard office equipment and systems:
phone, copier, and computer, including Microsoft Office and the Google Suite.

Environmental Conditions:
Work is performed in an office setting with exposure to a Visual/Video Display Terminal (VDT) and extensive personal computer and phone usage. Some sitting, standing, bending, and reaching may be required. Some occasional travel is required for meetings and conferences.
