Wraparound Facilitator
The Wraparound Facilitator will work as part of the Child and Family Team to assist adolescents with emotional/behavioral needs (ages 5-21) and their families in strengthening systems of natural support, improving the ability and confidence of family members in managing their services and organizing formal and community supports through a comprehensive, individualized, holistic support plan. The Wraparound Supervisor supervises this position.
Primary Responsibilities
Engagement
  • Initiates contact with the family within (3) calendar days of receipt of referral
  • Conducts face-to-face with the family within (7) business days of assignment to discuss:
    • Wraparound goals
    • Wraparound process
    • Role of the referring agency
    • Expectations of Family/Caregiver
    • Expectations of child
    • Development of the Child/Youth and Family Team (CFT)
    • Client/Family/Caregiver safety issues
      • Creation of a crisis plan
      • Stability of housing, childcare and respite needs
  • Serves as the primary contact person for the family on a weekly basis at minimum
  • Convenes, coordinates, communicates and facilitates a Child and Family Team Meeting (CFTM) with the family and identified collateral contacts to implement the Plan of Care every 30 days at a minimum and sends updated Plans of Care to all team members with 72-hrs of updates
  • Assures that families have a voice, sustained access to, and ownership of their Plan of Care and associated resources
Documentation
  • In partnership with the family, develops an individualized Plan of Care every 30-days at a minimum that adheres to National Wraparound Initiative standards and treatment planning requirements consistent with 42 CFR 438.208(c)(3)
  • Submits the Plan of Care to the Statewide Management Organization (SMO) for review prior to the end of the initial 30-day authorization period
  • Works directly with the child/youth and family to implement elements of the Plan of Care
  • Formalizes, monitors and modifies the Plan of Care developed by the wraparound team and updates the Plan of Care to reflect the changing needs of the child/youth within 48-hours of every CFTM.
  • Coordinates the delivery of available services, including services reimbursable under 42 USC 1396d(a) educational, social or other services
  • Develops, in concert with the individual and collateral contacts, a transition plan when the child/youth has achieved the goals of the plan of care
  • Monitors the Plan of Care to ascertain whether certain services are being provided in accordance with the Plan of Care and are adequate
  • All contacts and updates to the Plan of Care must be reflected in the Electronic Health Record System within 48-hours
  • Complies with all other required documentation and data collection in support of the program’s implementation and evaluation strategy
Collaboration
  • Works with the Independent Evaluator (Licensed Mental Health Professional) each eligibility period to ensure that the comprehensive assessment is completed, and the results are incorporated into the CFT process
  • Collaborates with other service providers on the child/youth and family’s behalf on an ongoing basis
  • Demonstrates an ongoing awareness of the community resources that could potentially be useful to the child/youth and family
  • Identifies other community and individual supports that can be used by the family to achieve their care plan; these can include friends, relatives and or religious/community affiliations
  • Develops positive relationships with community-based providers to ensure access to quality services for children youth and families
  • Ensures that an established community crisis support service is available to family during times of crisis, regardless of the time of day or day of week
  • Assists the child/youth and family with accessing strength-based mental health, social services, educational services, and other supports and resources required to help the family to attain its vision
  • Other duties as assigned.
Required
  • Possess a Bachelor’s-level degree in a human services field or any field with a minimum of two years of full-time experience working in relevant family, children/youth, or community service capacity. Relevant alternative experience may substitute for the Bachelor’s-level degree requirement in individual cases subject to approval by OBH
  • Completes required training and credentialing process for wrap facilitation according to a curriculum approved by OBH within time frames required by OBH
  • Demonstration of high fidelity to NWI standards through ongoing participation in wraparound fidelity monitoring using the WFAS
  • Access to reliable transportation. If driving an automobile while on company business, must possess a valid driver’s license in the state of residence and auto insurance
  • Ability to create effective relationships with individuals of different cultural beliefs and lifestyles
  • Possess an understanding of child and adolescent development
  • Strong interpersonal skills and the ability to work effectively with a wide range of constituencies in a diverse community
  • Effective verbal and written communication skills
  • Ability to develop and deliver a child/youth/family specific presentation
  • Strong organizational skills and detail-oriented
  • Ability to analyze complex information and to define and solve problems
  • Maintain confidentiality in all aspects of client (i.e., HIPPA, FERPA, P.L. 46:56) staff and agency information
