Job description

Mountain Care Network is a West Virginia-based licensed behavioral health agency seeking to improve the lives of fellow West Virginians. We are licensed clinicians in our communities providing quality mental health services to children, adolescents, adults, and families in Kanawha, Putnam, Boone, Mason, and Lincoln Counties.

We are seeking a passionate and energetic, Bachelor's level mental health professional to join our team as a Wraparound Facilitator to provide services to children on the CSED Waiver. We want practitioners that are devoted to helping fix the problems our communities are currently facing.

The Wraparound Facilitator performs the majority of tasks related to case management services needed for a child or youth who have been approved to receive Chapter 502 Children with Serious Emotional Disorder (CSED) Waiver Services. The primary goals of the intervention are to support family strengths, promote community involvement and ownership, provide the needed resources to families to improve family well-being, and reduce need for intensive psychiatric in-patient care. The CSED Wraparound Facilitator will coordinate, create and individualize services and supports to fit the unique need of the child and family while building upon their strengths. This position does not have any supervisory responsibilities.

Essential job duties include:

· Provide case management services for a caseload of up to 15 members

· Coordinate a comprehensive set of supports, resources, and strategies for each member and family

· Plan and facilitate team meetings with the youth, family, and service providers

· Ensure services and clinical treatment modalities augment each other for optimal outcomes

· Work with the MCO care manager to identify service providers, natural supports, and other community resources to meet member and guardian needs

· Lead the planning team through engagement and team preparation, initial plan development, plan implementation and implementation of a transition plan

· Administer the Child and Adolescent Needs and Strengths assessment tool

· Monitor the implementation of the plan, making sure the member and the guardian are receiving services identified in the plan

· Initiate and coordinates discharge and after-care planning

· Provide linkage and referral to services and supports as identified in the plan

· Conduct home visits with youth and / or family in accordance with the needs determined in the individualized plan

· Identify and develop formal and informal services / resources available to youth, family, and community

· Identify service gaps and collaborate with community stakeholders and existing services provider to resolve them

· Facilitate the development and implementation of an individualized transition plan for members to will reach the waiver’s maximum age limit

· Ensure documentation is thorough, accurate and timely

· Ensure that pertinent data is collected and compiled as required

· Maintain client records in compliance with organizational goals and objective as well as regulatory guidelines

Education and Expereince Requirments:

· Bachelor's degree (required) in a human services field (preferred)

· 2 years experience in social services or behavioral health (preferred with human services degree; mandatory if degree is not in human services)

Pay: $33,000.00 - $40,000.00 per year
