We are seeking skilled and compassionate Nurse Practitioners with specialized expertise in Wound Care to join our dynamic team at Vitality Advanced Healthcare. Nurse Practitioners play a crucial role in assessing, treating, and managing patients with acute and chronic wounds. This position offers an exciting opportunity to make a significant impact on patients' lives through innovative Wound Care practices.


Work Location: Hybrid


Work Shifts: Flexible - PRN


Responsibilities:


  • Perform thorough assessments of patients with acute and chronic wounds, considering medical history, current health status, and wound characteristics.
  • Develop and implement personalized Wound Care plans in collaboration with the interdisciplinary healthcare team.
    Provide evidence-based Wound Care treatments, including wound cleaning, debridement, dressings, and other advanced Wound Care modalities.
  • Monitor and track the progress of wound healing, adjusting treatment plans as needed for optimal outcomes.
  • Educate patients and their families on Wound Care techniques, prevention strategies, and lifestyle modifications to support healing.
  • Collaborate with physicians, nurses, and other healthcare professionals to ensure comprehensive and coordinated patient care.
  • Maintain accurate and detailed patient records, documenting assessments, interventions, and progress notes.
  • Stay up-to-date with the latest advancements in Wound Care practices and incorporate them into patient care strategies.


Qualifications:


  • Master's or Doctorate degree in Nursing from an accredited program.
  • Current valid Nurse Practitioner license in Arizona.
  • Certification as a Wound Care Nurse Practitioner (WOCN) or (WCC) - Preferred
  • Proven experience in Wound Care assessment, treatment, and management.
  • Strong clinical judgement and critical thinking skills.
  • Excellent communication and interpersonal abilities to effectively collaborate with patients, families, and healthcare professionals.
  • Ability to thrive in a fast-paced, team-oriented healthcare environment.
  • Excellent Communication & Customer Service Skills
  • Must have a valid driver’s license and auto insurance (local travel is required)
