We are looking for a Medical Case Manager Supervisor to join our team at our Emerald clinic.
Purpose
The Wellness Medical Case Manager Supervisor shall be accountable for 1) Wellness Medical Case Management Services day to day operations, 2) coordinating schedules to ensure adequate staffing, 3) successfully supervising the case management team.
The Wellness Medical Case Manager Supervisor is an important team member supporting the Wellness Center and Emerald Clinic, which is essential for achieving the dual mission of Full Circle Health. The WMCM Supervisor shall be accountable for 1) Understanding and complying with the Ryan White Funding expectations and requirements, 2) Providing oversight, training, and management of medical case management staff and activities, and 3) Maintain an understanding of HIV treatment and care guidelines.
The WCM works closely with Wellness Center Staff, as well as team members from Emerald nursing and reception, pharmacy, and providers.
As a leader within Full Circle Health the WMCM Supervisor must possess the professional leadership characteristics, must demonstrate the value system, and must have the managerial and technical skills necessary to support organizational and departmental goals. Full Circle Health leaders are critically important to the creation and maintenance of a positive, collaborative, effective and professional organizational environment. This position is essential for achieving the dual mission of Full Circle Health.

8am to 5pm Monday-Friday daytime shifts

Education:
- Degree in Social Sciences, Nursing, Social Work, Psychology or related field from an accredited four-year college or university, preferred.
Experience:
- 3 or more years working in a social service organization or healthcare setting providing service delivery.
- 2 or more years working in a supervisory role.
Licensure / Certification:
- Chronic Care Management Certificate, required within 4 months of hire.
Other Knowledge, Skills and Abilities:
- Excellent verbal and written communication skills
- Bilingual, preferred.
