Responsibilities, Deliverables, Outcomes and Expectations


• Assist clients in articulating personal and professional goals supporting recovery through the use of one-to-one and group sessions. During these sessions the Wellness Mentor will support clients in identifying and creating goals, which are compatible with active recovery plans, to build work skills, identify strengths, provide support, and offer resources to aid the client in achieving those goals.
• Be able to follow a clients Individualized Service Plan.
• Assist clients in working with their case manager or treatment team in determining the steps he/she needs to take in order to achieve these goals and self-directed recovery.
• Assist clients in setting up and sustaining self-help (mutual support) groups, as well as means of locating and joining existing groups.
• Support clients’ vocational choices and assist them in choosing a job matching their strengths, overcoming job-related anxiety by reviewing job applications, and providing interview tips.
• Assist clients in building social skills in the community enhancing job acquisition and tenure.
• Responsible for explaining the concept of the OMJ Pike County Career Center to the client and outlining the variety of available self-directed workforce development services.

• Responsible for establishing and maintaining client case management files, including follow-up and reports for all clients assigned and making appropriate referrals to partners as needed.
• When applicable, independently or with periodic assistance of program team members, utilize and teach problem solving techniques with individuals; discussions will be utilized where clients will share common problems in daily living and methods they have employed to manage and cope with these problems. As one who has availed themselves to mental health services, the PS will share their own experiences and what skills, strengths, supports, and resources they use. If applicable, the PS will share their own recovery story or personal success in and as the facilitator of these sessions, will demonstrate how they have directed their own recovery.
• Use ongoing individual sessions to teach clients how to identify and combat negative self-talk and how to identify and overcome fears by providing a forum which allows clients to share their experiences.

• Ability to handle confidential information in a professional manner
• Responsible for reading, routing, and drafting documents as needed
• Ability to remain calm in times of crisis.
• Responsible for being familiar with all office Policies and Procedures.
• Responsible for completing weekly/monthly reports as needed
• Organize and Maintain files as needed
• Responsible for developing and maintaining good rapport with local industry, businesses, social service organizations, and training institutions
• Responsible for performing outreach and recruitment, participating in bi-monthly Transitions Program Meetings, and improving local awareness of the Transitions Program and the local workforce development system by making group presentations about services and programs available
• Ability to maintain a clean workspace
• Committee Assignments and other duties as assigned by supervisor

 

Minimum Requirements…

 

Education:
Certified Peer Recovery Supporter (CPRS) or equivalent certificate or must obtain certification within Community Action’s 180-day instructional period.
High School Diploma or equivalent

 

Job-Related Experience:
Minimum of 2 years’ experience in areas of direct patient care, or Peer Recovery support
Experience with Microsoft Office Suite, specifically Outlook email and calendar.

 

Agency Requirements:
Must have excellent organizational skills with the ability to prioritize completing tasks and projects.
Must possess a valid Drivers’ License, have reliable transportation, be insurable under agency plan and have up-to-date proof of insurance.
Licenses, Certifications, and Accreditations:
Certified Peer Recovery Supporter (CPRS) or equivalent

 

Preferred Qualifications…
• Chemical Dependency Counselor Assistant (CDCA) Certification preferred.
• Two (2) years’ experience in office setting
• Working knowledge of Pike County, as well as the surrounding areas including employers, social services, and the Wellness, employment, and training resources available within the areas.

 

Working Conditions…
• Position is in a well-lighted office environment. This position requires occasional local and regional travel (up to 40%) for the purpose of engaging with the assigned programs/departments. Occasional evening and weekend travel/work is needed. Work is fast paced, and involves multiple, sometimes competing priorities, and the management of complex project details. Can involve moderate-to-high levels of stress.
