Job Description: Veteran Case Manager
Part Time
Primary Location: San Angelo, TX
Travel Requirement: Frequent day travel with less than 3% overnight travel
Hours: 08:00 to 17:00, with some variability as necessary to conduct outreach to veterans within the service area.
Reports to the Director of Veteran Services
Objectives of the Veteran Case Manager program: Our mission is to reduce barriers and ensure veterans, service members, and their families reach their full potential in mind, body, and spirit, using evidence-based counseling, educational, and case management services. Provide outreach to veterans and their immediate family members within the WTCG service area and linkage to services offered by the Veteran Services Collaboration.
Qualifications:
1. Experience as a service member, veteran, and/or family member (SMVF).
2. At least one year of experience working in the human services, with preference given to experience with military and/or veteran populations;
3. Knowledge about community resources and social services;
4. Proven interpersonal skills and effective as a member of a team;
5. Ability to maintain relationships with client base and community stakeholders;
6. Strong public speaking and writing skills;
7. Training in suicide prevention and crisis intervention (can obtain after hire);
8. Experience with tracking data and timely data reporting;
9. Experience documenting service provision;
10. Established case management skills;
11. Knowledge of Veterans Affairs and other veteran service programs;
12. Proficient in use of Microsoft Office, specifically Word, Excel, PowerPoint, & Outlook;
13. Must pass comprehensive background check;
14. Must have driver’s license and reliable transportation, and be insurable; and,
15. Preference given to applicants with a degree in a human services related discipline (social work, psychology, sociology, etc).
Duties
1. Serve as the initial point of contact for veterans and their natural supports and as the liaison between existing local veterans’ service organizations and WTCG veteran outreach services.
2. Engage in program participant intake and service coordination, to include:
a. Referral to the Veteran Services Collaboration via community outreach
b. Gathering necessary identifying information from each client served, to include demographics and verification of service/relationship to veteran
c. Assisting program participants in setting appointments with providers within the Veteran Services Collaboration upon intake
3. Gain familiarization with staff specialties for proper matching/referral of program participants.
4. Serve as liaison to Goodfellow AFB.
5. Provide community education, marketing, and outreach related to Veteran Outreach Services to the fourteen counties served and garner the respect of peers and the local veteran population in this effort.
6. Facilitate and recruit participants for the veteran and family support/treatment groups.
7. Documentation of services provided
8. Assist with reporting requirements as necessary for grantor and funding organizations.
9. Refer veterans to available programs and assist them as needed.
10. Provide ongoing case management services to participants of the program.
Job Type: Part-time
Pay: $19.00 - $22.00 per hour
Expected hours: 18 – 22 per week
