VBC Care Team Navigator is theone point of contact for all external stakeholders to support customer service standards and operational excellence.
Responsibilities include:
- Proactively feeds forward information between a multitude of stakeholders.
- Monitors VBC team voicemail box and alerts appropriate team members for follow-up.
- Serves as the primary contact liaison between the physician practices and the VBC team.
- Inputs data from clinical team members.
- Assists the VBC team, physicians, and APPs with workflow documentation completion.
- Tracks patients through the acute care process until discharge and alerts appropriate Care Team members of the patients' ongoing location status.
- Provides utmost customer service to stakeholders.
- Maintains a positive attitude and desire to serve.
- Preps charts, follows timelines, and addresses performance for documentation completion.
- Complies with regulatory and legal standards.
- May be asked to round directly in physician practices from time to time.
- May be asked to speak to patients from time to time.
- Perform other duties and responsibilities as required, assigned, or requested.
Qualifications:
- HS diploma, associate's, or bachelor's with healthcare customer experience or executive assistant experience preferred.
- 1-2 years of related experience.
- Expert computers skills with the ability to navigate and filter multiple software platforms at once.
- Customer service acumen.
- This is primarily a work from home position. Employee must have a quiet work from home space and be free of personal distractions during normal business hours.
