Value Based Care Quality Coordinator

As part of CMS’ Value Based Care Program, there are several quality metrics that are monitored to ensure the best and most cost-effective care is provided to these patients. NANI is seeking a Quality Coordinator to help monitor, track and ensure compliance with these quality measures.

Summary: The Quality Coordinator would be responsible for the maintenance of multiple reports and patient outreach related to quality metrics as designated by CMS for the CKCC Value Based Care program.

Essential Functions:

·         Maintains and updates the appropriate program software to manage and record required information and data.

·         Generates and analyzes reports as needed for management, identifying trends, anomalies, and areas of concern.

·         Collaborates with all levels of the clinical team to improve processes, communication, and team skills needed to provide the best care to our patients.

·         Utilize clinical judgement, independent analysis, critical thinking skills, time management skills and detailed knowledge of care coordination.

·         Actively participates in committees/meetings as needed.

·         Manage and track multiple complex patients and initiatives at once

·         Communicates with patients as need for coordination of care

·         Travel is required to our various offices when needed.  Our offices extend from Lake County, IL to Fort Wayne, Indiana.  Travel expectation is minimal but still a possibility.

Minimum Qualifications/Education:

·         Medical Assistant or an equivalent combination of experience

·         2-5 years’ previous experience in healthcare, preferably nephrology

Preferred Qualifications:

·         Strong computer skills and proficiency in MS Office and Outlook, especially Excel

·         Excels at developing strong patient and family relationships that fosters engagement and best outcomes

·         Minimum of 3 years’ experience in renal, ER, ICU, or Ambulatory Care settings

·         Demonstrated knowledge and understanding of data and managing clinical, financial and patient satisfaction outcomes

·         Proficiency with documenting and navigating electronic medical records (EMR), especially Acumen Epic
