Job DescriptionHours: M-F 9:00 to 5:00Department: Medical ServicesHome Base: John Bell Health CenterSupervisor: Advance Practice Provider (PA/NP)Full-Time, 40 hours per week; Exempt or Part TimeThe function of the Urgent Care Advance Practice Provider (PA/NP) is to provide Urgent Care for existing FIGHT patients whose treatment needs may be complicated by HIV, HCV, substance use disorder, and other vulnerabilities including race, ethnicity, language, immigration status, and LGBTQIA+ status at Philadelphia FIGHT's FQHC Community Health Centers. The Urgent Care Advance Practice Provider (PA/NP) will be responsible for providing walk-in and same day clinical care.
Job Responsibilities
Responsibilities include:
Primary Care: Supervise and coordinate Urgent Care for patients doing walk-in visits at the Philadelphia FIGHT Community Health Centers. Participate in both acute care and "lapsed" walk-in chronic care visits. The provider will insure that patients seen during walk-in visits have appropriate follow-up scheduled with their primary care provider. Assure that all care is delivered in a manner that is compliant with all applicable laws and regulations, the requirements of payers, and consistent with all policies that affect Philadelphia FIGHT as an FQHC. The Urgent Care Advance Practice Provider (PA/NP) will remain knowledgeable of applicable standards of care and treatment guidelines and will maintain continuing education credits as required by his, her or their professional licensure, to remain up to date on all such standards and guidelines. They must have the ability to understand patient needs as consumers of health care and focus service around those needs.
Qualifications include:
Knowledge and Education: The Urgent Care Advance Practice Provider (PA/NP) will be a licensed clinician with preferably three or more years post training experience. Experience in providing medical care to underserved populations is required and experience in HCV, substance abuse treatment and PrEP care are preferred. The Urgent Care Advance Practice Provider (PA/NP) will maintain up to date licensure in the state of Pennsylvania, and continuing education credits at all times. CPR certification is required.
Skills: The Urgent Care Advance Practice Provider (PA/NP) needs to have highly effective oral and written communication skills and have the ability to relate to and work with people of all ages, social and ethnic backgrounds. The physician must have the ability to work independently and as part of a team and inspire confidence and trust in patients and staff and have the ability to understand patient needs as consumers of health care and focus service around those needs. Additionally, the ability and willingness to obtain hospital privileges is required.
Substance Abuse Treatment: The Urgent Care Advance Practice Provider (PA/NP) will be required to prescribe buprenorphine when appropriate. The Urgent Care Advance Practice Provider (PA/NP) will also work with a Harm Reduction team on regarding comprehensive Medication Assisted Treatment Program.
Clinic sessions: Establish regular clinic sessions of four hours' time eight sessions per week. Participate in after-hours call as a member of the clinical team. This provider is expected to take at-home, after hours calls for the clinic's 24-hour emergency line on a rotating basis - approximately 4 times annually (or every 3 months). This is an exempt position.
Quality assurance: Participate in regular case conference meeting, participate in ongoing peer review and other oversight protocols as necessary to assure that U.S. Public Health Service, DHHS, BPHC, HAB or other relevant guidelines are adhered to in the health center and that research conducted at the health centers by Advance Practice Provider (PA/NP) is in conformance with standards of Good Clinical Practice. The Urgent Care Advance Practice Provider (PA/NP) is also required to actively participate and be a clinical leader in quality improvement activities within the health center.
Confidentiality: They must maintain confidentiality of all patients, clients and research subjects of the FQHC in accordance with HIPAA.
Physical and Mental Requirements: Normal health center/office/community environment; see, hear, talk, walk, sit and or stand for long periods of time, reach, stoop, bend, lift up to 25 lb.; repetitive hand movement; use and view PC. Read and write; verbal and written communication, basic math skills, analyze, prioritize, meet deadlines, and maintain high standard of confidentiality. Use sound judgment. Move throughout the health centers and community.
Salary range: Competitive
Work Hours: 40 Hours/Week
Travel Requirements: Minimal local travel required between Philadelphia FIGHT Health Centers.
