Come join our growing team! The Care Team Home Health and Hospice is looking for a Full-Time Triage Nurse In Texas. This is a Remote position for a Registered Nurse/RN living in Texas. Prefer a candidate who can periodically report to the Houston office. We specialize in providing Home Health and Hospice in the home and facilities. KEY JOB RESPONSIBILITIES: Responsible for triaging and resolving issues for Hospice. Maintaining the Home Care/Home Base database for accuracy. Responding to the needs of the patients, families and physicians calling into Triage with any issues. Schedule field staff in an accurate and timely manner.

Triage Registered Nurse/RN Responsibilities:

  • Triage patients requiring urgent and emergent medical advice evenings, weekends and holidays via telephone.
  • Triage Registered Nurse/RN p rocess incoming hospice referrals
  • Triage Registered Nurse/RN a ssign Admission, evaluation, subsequent, and discharge visits within EMR.
  • Triage Registered Nurse/RN r eassign Admission, evaluation, subsequent, and discharge visits within EMR.
  • Triage Registered Nurse/RN r un scheduling reports.
  • Triage Registered Nurse/RN a ssist field staff with ordering durable medical equipment (DME) and patient supplies as needed.
  • Triage Registered Nurse/RN a ssist with start of care admission process, displaying proficiency in the identification of hospice eligibility requirements set forth by Medicare guidelines.
  • Triage Registered Nurse/RN e ducate both the patient and family on medication management, treatment protocols and end of life processes along with identifying comprehensive and safe understanding of caregiver ability to provide safe care.
  • Triage Registered Nurse/RN e ngage in effective interdisciplinary communication with various professions of the hospice care team to promote the most effective care for the patient and their family.
  • Triage Registered Nurse/RN p roficiently document all health assessments, physician orders and communication with the patient, their family and various medical professionals in a timely manner.
  • Triage Registered Nurse/RN m aintains patient confidentiality, including all HIPAA regulations.
  • Triage Registered Nurse/RN c ompliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety and emergency management.
  • Triage Registered Nurse/RN m anages Territory with minimal supervision based on forecasted needs, priorities and business strategy.

Qualifications

PROFESSIONAL EXPERIENCE/QUALIFICATIONS:

  • 1+ year of previous Hospice nursing experience.
  • Minimum of one year of experience in medical/surgical and one year of experience in critical or emergency care with specialized triage training.
  • Experience in managing clinical services
  • Strong written and verbal communication skills.
  • Excellent customer service skills.
  • Commitment to The Care Teams philosophy of care, values, vision and mission statements as an exemplary role model for all staff.
  • Must have at least one-year experience in the Hospice industry.
  • Knowledge of Home Health is preferred .

Skills
  • Must be willing to get licensed in PA, IN, and MI
  • Must be able to make good professional decisions quickly despite stressful atmosphere.
  • Ability to handle stressful and constantly changing conditions.

Regulatory Requirements
  • Graduate of an accredited school of nursing, with a bachelors degree preferred.
  • Current state license as a registered nurse in the state of Texas
  • Must pass a criminal background check & MVR check.
  • Completed health statement acknowledging ability to perform the duties of the position.
  • Valid state drivers license.
  • Proof of current automobile insurance.
  • TB testing per agency policy; (1 or 2 step TB skin test within 12 months of hire & annual TB symptom screening thereafter).
