Central Ohio Primary Care Physicians is seeking a highly motivated RN committed to patient center care with strong customer service skills to fill a full-time Nurse Triage position at our Professional Pediatrics of Hilliard office located in Hilliard, OH. The preferred candidate will provide exemplary telephonic triage care to COPC's pediatric patient panel, utilize their clinical decision making skills efficiently, refer the patient on to the next appropriate level of care and possess knowledge of working within an electronic health record system. This telephonic position will be first shift hours, Monday-Friday 8:00am-5:00pm.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
- Conduct telephone triage for patient health complaints using the electronic triage assessment tool.
- Navigate multiple electronic platforms to assess current health complaint, patients’ electronic chart, PCP appointment scheduling, and ancillary center scheduling.
- Document nursing triage notes into the EHR system, ECW, for clear and concise documentation.
- Document phone call details in tracking system for department data tracking and collection.
- Assess, identify, and close patient care gaps. Identify patient needs to establish plan of action or next steps.
- Develop positive and effective working relationship with COPC staff, offices, and physicians in order to deliver high quality patient care.

Education, Licensures & Certifications
Required: Bachelor’s degree in Nursing
Required: Active and unrestricted Registered Nurse license in the state of Ohio
Preferred: Experience in telephonic nursing triage
