POSITION SUMMARY

 

The Triage Clinician works as an integral part of Wheeler's Behavioral Health Access Team to help complete the following: the initial intake assessment, crisis assessment as needed, brief intervention and stabilization, as well as clinical and basic care coordination services throughout the lifespan. The Triage Clinician does not have a caseload. The Triage Clinician will foster positive working relationships with clients, families, internal providers and clinicians, educators, and outside key stakeholders and utilize a strength-based solution-focused model of care.  The Triage Clinician will use a team approach in working with clients to provide ethical care and support.

 

This position is eligible for a $10,000 sign-on (half upon start and half after one year of service)!

 

EDUCATION AND EXPERIENCE/QUALIFICATIONS

Master's degree in Marriage and Family Therapy, Social Work, or Counseling or Doctoral degree in Psychology.  Must have experience providing crisis services.

 

Must hold one of the following credentials: LCSW, LMFT, LPC , LPCA, LMFTA,or licensed Psychologist in the State of Connecticut.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Conducts screening and evaluation and formulates diagnosis in accordance with the Diagnostic Statistical Manual (DSM). Develops and documents, clear clinical formulations, crisis and treatment plans that are individualized and measurable, developed in collaboration with the client/family and others as relevant.

 

Demonstrates knowledge of psychopathology, crisis theory and other behavioral health concerns and primary modalities of psychotherapeutic intervention. Triages clinical emergencies and implements agency specific safety and consultation protocols. Utilizes best practices and evidenced based practice models where appropriate.

 

Demonstrates knowledge of internal and external levels of care, referral processes and resources. Makes clinically appropriate referrals for identified service needs in a timely manner.

 

Communicates in a positive, effective manner with client, family, and all internal and external providers. Attends any required meetings.

 

May work with emergency departments, inpatient units and congregate care providers to identify and coordinate crisis and diversionary planning for  high needs youth in need of intensive community based planning to prevent multiple admissions to such settings and otherwise promote the use of mobile crisis to coordinate intensive plans for high needs youth.

 

May participate in community based meetings and conference calls with emergency departments, inpatient units, congregate care providers, other mobile crisis providers and  the Department of Children and Families to assist in care planning and  to address system issues which may impact increased emergency department and inpatient unit utilization of high needs youth.

 

With assistance from Project Director and other leadership, may coordinate with other Behavioral Health Access Team members to provide follow-up care and stabilization to identify high needs patients.

 

Demonstrates knowledge of program/department/agency goals and target outcomes, and adheres to practice standards and to data collection and reporting protocols.

 

Documents all client related information in compliance with agency policy and contractor requirements.

 

Attends all Behavioral Health Access Team trainings as required by the Behavioral Health Directors at Outpatient Behavioral Health Services at the Family Health & Wellness Centers.

 

Attends and is prepared for scheduled supervision. Seeks additional supervision or consultation as needed and follows through with supervisory directives.

 

Continues to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served as well as uses appropriate methodological approaches, skills, and techniques that reflect an understanding of culture.
