Form of workPosition Summary: Coordinates the MDS and Care Planning process to ensure timely and accurate submission with the Interdisciplinary team.

Position Responsibilities:
* Must follow HCF Policies and Procedures
* Must meet job related competencies as outlined in the Skills Competency Manual
* Assist with nursing staff education in the documentation, MDS, CAAs and Care Plan process
* Must accurately complete MDS assessments in accordance with established time frames for new admissions, quarterly, annual and significant change
* Must pre-check and electronically submit MDS’s to State agency, review validation reports and make necessary changes
* Coordinates the scheduling and timely completion of MDS’s and Care Plans with the Case Manager and Interdisciplinary team
* Able to electronically access, print and evaluate Quality Indicators
* Assumes responsibility for maintaining his/her own professional competence
* Perform other duties as assigned
Knowledge, Skills and Abilities: Graduation from an accredited school of Nursing. Current license as a Registered Nurse with the State Board of Nursing. CPR certification recommended. A Licensed Practical Nurse may practice as a MDS/RNAC under the direction of a RN within the state of Ohio.
Supervisory Responsibilities: There are no supervisory responsibilities.
Personal Appearance: You must maintain a professional appearance by keeping neat, clean and well groomed at all times. Must wear white pants, professional appearing smock and predominately white rubber soled shoes and ensure their neatness and cleanliness. Jewelry must be conservative in nature in order to reduce the risk of resident and employee injury. Hair should be neat, pulled back and away from face. Nails should be neat, clean and trimmed. Must wear identification name badge.
