REQUIREMENTS:

• Minimum of 2 Years of Experience in Outpatient Care
• Active Valid Driver's License

CERTIFICATION REQUIREMENTS

• BLS

LICENSURE REQUIREMENT

• Oregon
• Can Accept Any Active State License
