TARLANI Healthcare is currently seeking experienced and dedicated Registered Nurses to join our Home Health team. This skilled nurse will be able to make visits to the patient?s place of residence, to provide skilled care services as ordered.
Per-Diem status only (Skilled Nurses are able to choose areas of their choice).
Essential Job Functions/Responsibilities
- Graduate of a nursing school.
- Must have a minimum 1 year of experience in Nursing
- Must be open to working a minimum 3 days out of the week (Sunday through Saturday)
- Experience in EMR software (preferred)
- Uses health assessment data to determine nursing diagnosis.
- Must complete an initial assessment of patient and family to determine home care needs. Provide a complete physical assessment and history of current and previous illness(es).
- Assess patient needs, evaluate and modify care to meet goals of skilled nursing interventions.
- Collaborate with all case managers and DON to plan and evaluate team goals for each patient.
- Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
- Submit reports and documentation for each patient in accordance company?s deadlines.
