Pay: $18.94/ Hourly

About the job:

The Transportation Scheduler is responsible for scheduling internal and external medical appointments for the Senior Community Care participants and notification of appointments with participants, their family and home care providers.  

Required Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  • High school diploma or equivalent required
  • Have a minimum of one-year experience in working with the frail elderly
  • Must pass a background check and drug screen.
  • Must have medical clearance for communicable diseases and up-to-date communications before having direct participant contact.

Preferred Skills:

  • Current License as Certified Nursing Assistant (C.N.A) preferred.

Essentials: 

  • Considers the needs of the participant and requirements of the PACE Center.  Communicates these changes to Nursing Services Department.
  • Coordinate scheduling of Transportation with Transportation Dispatcher for participants to medical/specialist appointments, and Day Center, as requested by the Interdisciplinary Team via the plan of care.
  • Assists in scheduling vehicle maintenance and equipment to operate within manufactures recommendations.
  • Functions as a member of the Interdisciplinary Team when appropriate.
  • Familiar with Transportation Para Plan Scheduling Program to be able to gather information needed for timely and efficient individual transportation and home care schedules.
  • Follows all Senior Community Care policies and procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.
