Day (United States of America) The Nurse Coordinator role is now eligible for a sign on bonus.
Important Details: Bonuses are paid out in 3 payments: at 30 days of hire, 6 months of hire and 1 year of hire. To be eligible, you must be a non-BILH employee or a previous eligible employee who returns to BILH after 1 year. Employee must be in good standing to receive the bonus at the time of payment. All bonuses are subject to applicable taxes. If you leave Lahey Health & Medical Center before your first anniversary you will pay back the sign-on bonus to Lahey Health & Medical Center. This program is subject to change at any point.
Under the general direction of the Chair, Department of Transplantation and Hepatobiliary Diseases, the Administrative Director, and the Associate Chief, Ambulatory Nursing Practice, and within the Lahey Clinic Professional Practice Model, MA Nurse Practice Act and ANA Standards of Practice and Code of Ethics the Transplant Nurse Coordinator is responsible for providing nursing care, education and support to organ donors and transplant candidates, patients on the pre-transplant waiting list and post-transplant patients both in the hospital and in the community. A Transplant Nurse Coordinator collaborates in the delivery of health care services with other team members including pharmacy, social service, nutrition, radiology, physical therapy, laboratory and case management. A Transplant Nurse Coordinator interacts with patients in the clinic, performs nursing assessments and consults with other care providers in developing treatment plans. A Transplant Nurse Coordinator provides telephone triage for pre and post transplant patients and live donors. Patient, family, and nursing colleague education is an integral part of the Transplant Nurse Coordinator’s role.

Job Description:

Essential Duties & Responsibilities including but not limited to:

1. Utilizes the nursing process to promote quality patient care.
2. Develops and maintains a therapeutic relationship and continuing involvement with patients.
3. Coordinates and collaborates with all health care professionals and community resources to ensure that patients benefit from continuity of care and receive all necessary services
4. Develops teaching tools for transplant candidates, recipients, living donors, social supports/families and nursing colleagues.
5. Acts as a nursing consultant on evidence based practice in transplantation for colleagues throughout the organization.
6. Participates in the education of inpatient nursing colleagues in the area of normal and abnormal organ function, transplantation and organ donation with emphasis on identifying nursing problems.
7. Participates in appropriate nursing service and hospital committees.
8. Conducts nursing research and prepares articles for publication in professional journals.
9. Collaborates with the multidisciplinary team about treatment decisions.
10. Collaborates with nursing and physician leadership in establishing policies and procedures for the transplant program.
11. Participates in unit quality improvement activities and is familiar with unit based metrics.
12. Maintains a high level of professional standards by participating in continuing education, professional organizations and community outreach.
13. Achieves and maintains certification as a Certified Clinical Transplant Coordinator according to organizational and/or departmental standards.
14. Handles staff and patient confidential information in strict conformance with department/institutional policy.
15. Delegates work appropriately and provides direction and guidance to nursing and support staff.
16. Documents relevant clinical issues in the transplant database, inpatient or outpatient medical records, and in other regulatory required database systems.

Minimum Qualifications:

Education: Bachelor of Science Degree in Nursing from an accredited school of professional nursing.

Licensure, Certification, Registration: Current Massachusetts state nursing license. Current certificate of Basic Life Support (BLS).

Skills, Knowledge & Abilities:

5. Ability to organize and work in a multi-task environment.
6. Computer skills including Database and word-processing.

a. Demonstrates the necessary clinical skills and technical knowledge to provide care for patients according to Nursing Policy and Procedures.
b. Demonstrates the necessary clinical skills and knowledge to provide care for patients according to the unit-specific competencies.
c. Ability to cross train to other clinical areas.
Technical Skills
a. Demonstrate the necessary clinical skills and technical knowledge to provide care for patients according to Nursing Policy and Procedure.


Experience: Two years of transplant related nursing experience
