We are seeking a Transitional Planner I to join our ReentryWorks unit. ReentryWorks seeks to improve reentry outcomes for people returning to NYC by assessing their specialized needs and connecting them with appropriate services prior to their release from NYS prisons. This role would assist with the connection to NYC services, including logistical support for release days, and by providing release day transportation for individuals released within the Hudson Valley and transporting them to NYC.

This role is based in our Newburgh office but would be a full time, hybrid work schedule, with 1-2 days per week, picking up participants from a nearby NYS prison and transporting them to NYC, 2 days in our Newburgh office, and the rest remote work.

Salary range: $26.10

Requirements:

Essential Duties:

  • Coordinate with community service providers, Department of Corrections and Community Supervision, and participant family members to finalize the transitional plan and make necessary referrals with actual appointments in the community
  • Assist in drafting comprehensive transitional plans for participants
  • On release day, assist participants with release day logistics and troubleshooting, including being available Monday to Thursday, 8am-4pm for family members, DOCCS, Criminal Justice Reentry Network providers, or others calling for assistance with a participants' release
  • Transport participants from prison to their respective residences or program in New York City
  • Provide transportation of participants, staff, or materials to programs, trainings, other offices, etc., as needed
  • Provide motivational interviewing with participants as needed
  • Manage the ReentryWorks vehicle and arrange maintenance as needed

Minimum Qualifications:

  • High School Diploma or equivalent with at least one year of experience in social service, case management, or customer service
  • Valid New York State Driver’s License and clean driving record

Key Competencies:

  • Ability to communicate effectively with strong attention to detail, time management, and conflict-resolution skills
  • Ability to work in teams and collaborate effectively with people in different functions
  • Ability to travel to prisons within the Hudson Valley
  • Proficient in Google Suite and Microsoft Office
  • Bilingual Spanish is preferred
  • Have an understanding of the criminal justice environment
