The Therapist interacts with the treatment team and provides a variety of patient care activities daily. They are responsible for managing patient caseload, providing group-individual-and family therapy, conducting bio-psychosocial assessments and more! The Therapist collaborates with the treatment team on a daily basis, formulates the master treatment plan and assists in coordinating discharge planning while functioning under the guidance of their credentials and State/Federal regulations.
Essential Functions:
- Responsible for client caseload
- Provides group therapy, daily process group.
- Provides individual and family therapy.
- Formulates client's master treatment plan.
- Responsible for documenting progress notes and clinical interactions into client EMR
- Completes discharge paperwork in EMR in accordance with facility requirement.
- Conducts bio-psychosocial assessments.
- Collaborates with the treatment team in other departments.
- Attends clinical meetings.
- Acts in accordance with the regulations, rules, and requirements of State and Federal government and accrediting organizations.
Qualifications:
- Master’s degree in social work or a related major.
Preferences:
- LPC, LCPC, LCSW, LMFT, or LMHC in the state if IL
- CADC
- Work experience in the field of behavioral or mental health
