The Therapist interacts with the treatment team and provides a variety of patient care activities daily. They are responsible for managing patient caseload, providing group-individual-and family therapy, conducting bio-psychosocial assessments and more! The Therapist collaborates with the treatment team on a daily basis, formulates the master treatment plan and assists in coordinating discharge planning while functioning under the guidance of their credentials and State/Federal regulations.

Essential Functions:

• Responsible for client caseload
• Provides group therapy, daily process group.
• Provides individual and family therapy.
• Formulates client's master treatment plan.
• Responsible for documenting progress notes and clinical interactions into client EMR
• Completes discharge paperwork in EMR in accordance with facility requirement.
• Conducts bio-psychosocial assessments.
• Collaborates with the treatment team in other departments.
• Attends clinical meetings.
• Acts in accordance with the regulations, rules, and requirements of State and Federal government and accrediting organizations.

Qualifications:

• Master’s degree in social work or a related major.

Preferences:

• LPC, LCPC, LCSW, LMFT, or LMHC in the state if IL
• CADC
• Work experience in the field of behavioral or mental health
