This posting if for one temporary 20-hour week, contract Social Worker to provide Case Management services to at High-Risk pregnant women of Alamance County.This service includes assessment, planning, facilitation, care coordination, evaluation and patient advocacy to address health care and preventative service needs of pregnant and postpartum women. Care Coordination focuses on coordination of the patients' medical, nutritional, educational, social and emotional needs with private and community resources to promote healthy pregnancy outcomes. Community visits and patient home visits are best practice for meeting program measures.
This position will also be required to staff emergency shelters as mandated by the State Public Health Office; assist with Public Health outbreak investigations and response, and perform additional duties as required by the Nursing Supervisor, Nursing Director, Medical Director and Health Director

ESSENTIAL KNOWLEDGE, SKILLS, AND ABILITIES:
Thorough knowledge of principles, techniques, practices and application or social work relative to individual, group, and community casework: includes knowledge of psychosocial, socio-economic, and behavioral issues and treatment in addition to relative laws, and regulations, and policies governing social work practice and public health programs. Ability to function as a part of a multidisciplinary health care team, have knowledge of community resources and public assistance programs. Ability to express ideas clearly and concisely, in writing and verbally. Ability to plan and work independently.
MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS:
Bachelor's or Master's Degree in social work from an accredited School of Social Work.
NECESSARY SPECIAL QUALIFICATIONS: Valid N.C. driver's license and have access to a personal vehicle. License to practice as a registered nurse in NC preferred.
PREFERRED QUALIFICATIONS: Knowledge and experience with Maternal-Infant Health/Pediatrics preferred. Fluent in both Spanish and English both oral and written.
