Temporary Case Manager

DSSI is hiring Experienced Case Managers to work with a workforce services company in the Chula Vista area. This is a temporary role assisting on a special project aiming to help those currently facing a housing crisis.

Details:

• Workdays: Monday-Sunday
• Work Hours: 7am-8pm
• Must have a flexible schedule
• Pay: $32/HR
• Start: ASAP

Duties:

• Meets with participants to identify barriers and track process of housing progress.
• Meet with clients in the community.
• Travel from site to site to assist in engaging clients
• Address client questions and concerns.
• Maintains complete, accurate and timely case notes and related records in manual and electronic systems.
• Track and reports out on participant progress and participation in assigned employment services activities
• Create, update and ensure completion of plans according to contract deadlines.
• Assess and refer participants to appropriate departments and/or agencies
• Complete all necessary case management documentation and forms; maintain records and files; and schedules appointments
• Other duties as assigned
• Bilingual/Spanish a plus
