Legacy Community Health is a premium, Federally Qualified Health Center (FQHC) that provides comprehensive care to community members regardless of their ability to pay. Our goal is to treat the entire patient while improving their overall wellness and quality of life, in addition to providing free pregnancy tests, HIV/AIDS screening. At Legacy, we empower patients to lead better lives by promoting healthy behaviors and offering resources such as literacy classes, family planning services, and nutrition and weight management information.
Our roots began in 1981 as the Montrose Clinic, with specialization in HIV education, testing, and treatment. Since then, the agency has expanded to 10 clinics in Houston, one in Baytown, two in Beaumont, and one in Deer Park with extensive services that include: Adult primary care, HIV/AIDS care, pediatrics, OB/GYN and maternity, dental, vision and behavioral health. We also service students within KIPP and YES Prep schools. Legacy is committed to driving healthy change in our communities.
Job Description
The RN Telephone Triage Nurse will adhere to established policies and standards of care and practice at Legacy Community Health Services. The Telephone Triage Nurse is accountable for indirect patient care through telephone inquiries from patients who are seeking medical information and/or medical attention regarding their condition. The Triage Nurse will be able to answer questions, schedule appointments as the patient's condition requires and provide limited health education as per established protocols, policies, the Nurse Practice Act and other guidelines as applicable.
Essential Functions
  • Accept difficult, urgent or emergency calls from patients
  • Perform Telephone Triage using best practice guidelines and documentation to ensure patients receive the right level of care in the right setting at the right time by the right caregiver using the right equipment and supplies
  • Prioritize and address items in a timely manner and collaborate with other clinical team members on patient care
  • Gather necessary information and/or data for provider
  • Determine health care needs and make appropriate recommendations for intervention:
  • Provide nursing advice and patient education
  • Assist in coordination of care
  • Use approved protocols and/or other assessment tools during the provision of telephone advice
  • Provide consistent, appropriate and courteous telephone advice in a timely manner per practice protocols
  • Use the nursing process (assessment, diagnosis, outcome identification, planning, implementation, evaluation) in delivering safe care and accurately document care
  • Exercise judgment while coordinating care and referring patients to agencies or health care professionals for other services/transitions in care where indicated
  • Documents conversation in medical record 100% of the time
  • Safeguard patient privacy and confidentiality
  • Provide care in a non-judgmental, non-discriminatory manner, respecting the values of members of all cultures.
  • Provide care in a safe and cost-effective manner.
  • Establish professional relationships with peers and seek out colleagues for mutual information sharing
  • Maintain an average call time of 8 minutes or less
  • Maintain a pleasant, helpful and professional demeanor

Education & Training Requirements
  • Graduate of an accredited Registered Nursing program
  • Current Texas licensure as an RN
  • Current CPR certification required
  • Bilingual English/Spanish required
  • Ability to meet Legacy Community Health's EPIC proficiency requirements

Work Experience
  • Three (3) years of experience as an RN, preferably in family medicine
  • Previous nursing triage experience preferred
  • Demonstrated good clinical, interpersonal, organizational, written and verbal communication skills required
