JOB DEFINITION:
The OnTrackNY Program is a Coordinated Specialty Care (CSC) treatment program at CN Guidance and Counseling Services, which serves adolescents, and young adults who are experiencing first episode psychosis. Our mission is to empower young people to make meaning of their experiences and to pursue their goals for school, work and personal relationships. The Team Leader oversees all aspects of the team functioning and provides administrative and clinical oversight. In addition, coordinate outreach and recruitment activities for the team.
EDUCATION REQUIREMENT:Licensed Psychologist, Licensed Clinical/Masters Social Worker, Licensed Marriage and Family Therapist, Licensed Mental Health Counselor, or Licensed Creative Arts Therapist
EXPERIENCE REQUIRED:Three years' experience in mental health and/or chemical dependency field community- based setting.
DUTIES AND RESPONSIBILITIES:- Charged with supervisory and administrative responsibilities to ensure that staff meets the standards and requirements of the Agency and appropriate funding and regulatory
- Direct and supervise staff activities, lead team organizational and service planning meetings and provide direction to staff regarding individual cases. Conduct side-by- side contacts with staff and regularly conduct individual supervision meetings.
- Directly supervises all members of the OnTrackNY team including; Primary Clinician, Supported Education and Employment Specialist, Peer Specialist and Program Assistant. In addition, assists with supervision for Psychiatrist and Registered Nurse.
- Monitor case load size and coordinate program in takes and discharges
- Fosters and models principles that under lie the model (recovery, shared decision making/duty to care-dignity of risk, person centered, and cultural humility).
- Ensure all team members are kept informed of relevant issues involving individuals and their families and coordinate weekly team meetings.
- Responsible for over seeing Assessment and Service Planning, Quality assurance and Improvement Processes, Utilization Review, Incident Reporting, coordination between OnTrackNY and other systems.
- Engage in performing assessments, handling crisis intervention, discharge planning, safety planning and wellness management.
- Provide direct, face-to-face and/or collateral services to recipients either in the OnTrackNY office or in the community in places and situations where problems arise.
- Establishingwarmandcollaborativerelationshipswithcommunitypartnerswho might refer or provide ongoing support to young people and families.
- SpreadingawarenessaboutOnTrackNYservicesthroughcommunityeducationand outreach activities that emphasize OnTrackNY's uniquely flexible and youth-friendly
- Collaborate with OnTrackNY team members to set new community-based goals focused on reducing inequities and barriers to accessing specialty care.
- Identifyingandcreatingnewresourcestosupportdiverseyoungpeopleandfamilies seeking or receiving services at OnTrackNY.
- Ensuringawelcomingandefficientprocessforyoungpeopleandfamiliesseeking care in OnTrackNY.
- Servingasprimarycontactpersonforreferralsourcesandmaintainingon-going communication with referral sources.
- Provides psychoeducation regarding first episode psychosis (FEP)and the goals of the OnTrackNY program.
- Conducting telephone and in-person eligibility screening of young people referred to
- Referring participants in eligible for services to other appropriate community
- Facilitates weekly team meetings and provides training and individual supervision to team Primary Clinicians ,Supported Employment and Education Specialist, and Peer Specialist; and provides administrative supervision to the Prescriber and Nurse.
- Serves as the Primary Clinician for a subset of program participants
- Ensures staff attendance at required OnTrackNY trainings and consultation calls, and completion of additional OnTrackNY related training modules in the Center for Practice Innovations' Learning Management System (LMS).
- Must be available(along with other members of the OnTrackNY on an as needed basis 24 hours a day, 7 days a week. Manages the after-hour's calls for the team.
- Participates in planning for the needs of the community and the Agency with the rest of the administrative team.
- Support the use of data in clinical supervision to inform decision-making

- Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress
- Actively engage in agency wide trainings and work groups as well as OnTrackNY specific trainings.
- Identify and implement quality improvement trainings and activities as part of professional development goals
- Complete QI Training during onboarding and participate in QI refresher training as needed.
