Job Description
Community Counseling & Mediation (CCM), a Brooklyn-based behavioral health organization, is currently seeking a Case Manager for Treatment Access and Prevention Services program (TAPS) located at 535 Clinton Ave Brooklyn, NY. TAPS program is a case management outreach and prevention program funded by SAMHSA. TAPS is designed to provide outreach, HIV and Hepatitis testing, referrals to treatment, and case management services to males who engage in high-risk behaviors and their partners with substance abuse or co-occurring mental health disorders.
Responsibilities/Duties
- Provide prevention education, and awareness workshops to implement strategies to facilitate behavior change and reduce the stigma associated with HIV to the community at large at least monthly.
- Responsible for the provision of all case management services consist of intake, screening, assessment, home visit, treatment planning, counseling, couples therapy, substance abuse counseling, treatment adherence, group facilitation, harm reduction, HIV testing, and referrals to medical services, mental health services, housing services, Legal Services, and advocacy.
- Responsible for all documentation of client's progress toward agreed-upon goals.
- Will help develop clients' support systems and integrate them into community services that includes intensive outpatient treatment series, medication-assisted treatment, community support services, and recovery support services.
- On an as-needed basis, the Case Manager will conduct home visits, and ensure client's retention in services.
- Be trained in the provision of Evidence-based practices and ensure fidelity to them.
- Provide Prevention Navigation information to clients on PrEP and PEP.
- Refer clients for HIV and Hepatitis testing.
- Document and report on all outcomes and goals of the program to the Program Manager/Coordinator.
Qualification
- Master's degree in a related field.
- Two years of relevant experience.
- Knowledge of youth programming and substance abuse and co-occurring mental health disorders within racial minority populations and successful track record of providing services to at-risk youth and their families.
- Must be COVID-19 vaccinated with a booster.
Hours of Operation
10am – 6pm Monday through Friday with at least one evening until 8pm/Comp, some Saturdays/Comp as needed.
