SUMMARY
The Senior Vice President of Value‐Based Care Clinical Operations is accountable for designing and implementing clinical workflows, managing medical cost reduction initiatives, and overseeing the care management team. The role works closely with partners to innovate to reduce medical costs and drive clinical quality. This position works effectively across all key internal and external stakeholders and thrives in an ambiguous fast paced environment.
Essential Duties and Responsibilities include the following. Other duties and tasks may be assigned.
- Accountable for medical cost reduction initiatives across value-based care portfolio
- Accountable for designing and successfully implementing key clinical workflows: incl. transition of care; high risk of hospitalization; optimal start
- Works closely with nephrology practices to customize workflows as needed and ensure impact
- Measures effectiveness of interventions + continuous improvement
- Seen as thought leader and partner by key nephrologists
- Oversees Care Management team: incl. NPs, RNs, Pharmacists, Social Workers, Dieticians. Inspiring leader close to front line caregivers
- Clear roles and responsibilities
- Ensures successful roll-out of tools and continuous training / mentoring of IDT (interdisciplinary team)
- Effective management process (incl. IDT scorecard; process metrics)
- Ensures close coordination with dialysis clinics and clarity of roles / responsibilities.
- Works closely with dyad partner to: a) innovate and develop new initiatives to reduce medical costs and drive clinical quality; b) develop key external partnerships
- Initial focus will be on Medicare CKCC program. But expect SVP will play a key role in developing playbook and initiatives for private payor business in the future
- Upholds management goals of corporation by leading staff in team concepts and promoting a team effort.
- Maintains effective personnel management and employee relations, including evaluating the performance of all personnel; approving and submitting all hours worked and counseling and disciplining employees.
- Recruits, trains, develops, and supervises personnel.
- Actively promotes GUEST customer service standards; develop effective relationships at all levels of the organization.
- Effectively communicates expectations; accepts accountability and holds others accountable for performance.
- Regular and reliable attendance is required for the job.
Qualifications/Requirements:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
Requirements include:
- Bachelor's degree in related field required. Masters degree in nursing or related field strongly preferred
- Ten (10) plus years of leadership experience in a clinical setting
- Demonstrated experience building and implementing strategy to network and C-level contacts within health plan entities.
- Experience structuring and negotiating value-based contracts required.
- Proven ability to partner closely with physicians is required
- Knowledge of kidney care and/or End-Stage Renal Disease (ESRD) or similar field required
- Intermediate computer skills, including Microsoft Office (Word, Excel, PowerPoint, Outlook and web-based tools) required; proficiency in USRC applications required within 90 days of hire.
