Responsible for implementing PrEP and PEP program activities and tracking program outcomes including oversight of the Harm Reduction Program. Develops and monitors outreach plans to ensure program is meeting contracted goals, deliverables and maximizing our Maximum Reimbursement -MRA. Supervises a team including; Health Promoters, Benefits Navigator, Data Entry staff and Peer Navigators.
Responsibilities
- Supervises and trains members of the PrEP/PEP, Undetectables and Harm Reduction Program
- Develops, implements and updates a plan to recruit, engage and enroll patients into IMC's PrEP and PEP and Harm Reduction Program
- Provides EBP-team with, at minimum, Quarterly trainings and updates on EBP- Scope of Service.
- Conducts monthly staff meetings, which includes a discussion of monthly Service Targets
- Provides Administrative team with biweekly and monthly data reports on Service Targets for PEP/PrEP
- Creates, maintains and updates a QA plan to ensure program is meeting our Scope of Services and our Service Targets
- Oversees the prescreening of patients over the phone and scheduling patients' appointments in MediTech for follow-up PEP/ PrEP and Harm Reduction Services
- Oversees the staff that conducts assessments on clients/patients who are screened as being high risk for HIV infection and/or have a scored on high on the Mental Health and/or Substance Abuse screening tools.
- Oversees staff Conducting PrEP Re-assessments on each client enrolled into the PrEP program every 6 months.
- Ensures that electronic medical record reflects the documentation of program activities for the PrEP/PEP and Harm Reductions services and ensures that encounters are documented in Meditech MPM
- As necessary provides back-up to the Data staff to ensure services are completed and entered into eSHARE in a timely manner
- Completes and submits monthly ePNR reports to Public Health Solutions in a timely manner
- Supervises and monitors all data management and required reporting activities to ensure compliance.
- Participates in daily clinic huddles and Case Conferences to present any patient issues with the Interdisciplinary team.
- Orders PEP/PrEP starter medication deliveries from IMC pharmacy and maintains medication log book to track medication supplies.
- Schedules weekly Outreach events for team which includes the completion of HPlus screenings and identification of patients to enroll in the HIV Primary Care Program.
- Will attend all contract required NYCDOH and Public Health Solutions meetings and trainings and in-services.
- Develops partnerships and working relationships with organizations/agencies that provide referral and Supportive Services for the population served.
- Participates in all related quality/performance improvement activities.
- Attends all monthly Departmental staff meetings, QA meetings and Viral Load Suppression meetings.
- Perform other duties as required by the VP, Ambulatory Care.
Qualifications
Education:
- Bachelor's Degree in Health and Human Services or other related field required.
Experience:
- Experience using electronic health records.
Knowledge and Skills:
- Knowledge of HIV treatment education and PrEP/PEP services.
- Proficiency with Office PC's, Desktops, and Laptop software in addition to computer literacy with Microsoft Office softwares to include Word, Excel, PowerPoint, Publisher Outlook, etc.
- Good interpersonal skills a must.
- Ability to communicate effectively in English, both verbally and in writing.
- Strong verbal and written communication skills with peers and leadership.
- Good negotiation, collaboration, advocacy and teaching skills.
- Strong motivational and advocacy skills.
Physical Requirements:
- Involves standing, walking, sitting, talking, hearing and bending
- Work may require evening, night, holiday, or weekend assignments on occasion
- OSHA Category III - Exposure not anticipated. Work does not involve exposure to blood, body fluids, or tissue, but may require performing unplanned exposure tasks
- Position may require the use of safety equipment, such as HEPA mask, for infection prevention
