The Prince George's County Government's Department of Family Services is seeking qualified applicants to fill a Support Planner position Community Developer 1G), grade G18 within its Aging and Disabilities Services
About the Position:
The Prince George's County Department of Family Services is seeking qualified applicants to fill multiple Limited Term Grant Funded (LTGF) Community Developer I positions, grade G-18, within its Aging and Disability Services Division. This is entry level professional work responsible for advocating, counseling, assessing, developing, and providing community liaison/social service assistance to individuals interested in the Medicaid Home and Community Based Services Programs (Community Options, Community First choice, Community Personal Assistance Services, Money Follows the Person) and long-term care options. The incumbent may be required to work occasionally on some evenings or weekends. Work is performed and evaluated under close supervision of the Program Manager.

What You'll Do:
  • Provide professional social/work/case management services to individuals seeking enrollment in one of the Medicaid Home and Community Based Programs (Community Options, Community First Choice, Community Personal Assistance Services, Money Follows the Person)
  • Support Planners will provide program education on the Medicaid programs eligibility, and application/enrollment process.
  • Provide options counseling to nursing home residents and their guardians/family members interested in learning about services and supports in the community specifically related to Medicaid programs.
  • Provide ongoing case management services to include evaluation of the individuals need/medical assessment, assistance with development of care plans utilizing a person-centered approach.
  • Conducting quarterly visits to monitor and ensure that the services are being provided in accordance with the plan of service, progam policies, and procedures.
  • Make appropriate referrals for community resources based on client need, identifying Waiver and non-Waiver services.
  • Provide assistance to nursing home residents interested in transitioning to a community setting.
  • Assist with identifying affordable housing options and addressing housing related barriers.
  • Establish and maintain participants' information and case records in the Program database and ensure that all information is current and update.
  • Prepare case notes and other reports as required.
  • Collaborate with local Health Department, Dept of Social Services, Maryland Dept of Health and other community organizations concerning the operation of the Program and its participants.
  • Ability to take initiative, use sound judgement, exercise independent decision-making, and provide excellent customer service.

Qualifications
What You'll Need:
  • Bachelor's degree in social work, Psychology, Sociology, Public Health, Human Services, or other closely related field, plus responsible professional level work experience which includes 1 year case management, counseling, care coordination, and working with vulnerable populations.

Preferred Qualifications:
  • 4-year degree or higher to include at least 2 years of case management experience working with older adults and/or individuals with differing abilities, knowledge of Medicaid and Medicaid related services and/or Disabilities related services.

CONDITIONS OF EMPLOYMENT:
Must complete a thorough background investigation.
May be required to work occasionally on some evenings and weekends.
DURATION OF ELIGIBILITY: Candidates will be selected from a temporary register of eligible candidates, which will become effective approximately four (4) weeks after the closing date. Once a selection has been made, the register will expire.

General Plan Information:
The Prince George's County benefits plan year is from January 1 to December 31.
A spouse (to include a same sex spouse) can be added to the health benefit plans. A marriage certificate and social security number is required to add a spouse.
Children under the age of 26 are eligible for coverage under the health benefit plans. This includes stepchildren and children of the same-sex spouse. A birth certificate(s) and social security number(s) is required to add a child(ren). If you are only adding the stepchildren or child(ren) of a same-sex spouse, you will need to submit a marriage certificate. You will also need to submit the birth certificate of the child(ren) and your spouse must be listed as a parent.
The premiums for health benefits are deducted on a pre-tax basis with the exception of Long-Term Disability, Extra Life Insurance and Voluntary Benefits (Short-Term Disability, Whole Life Insurance, Critical Illness, Accident Insurance, Cancer Indemnity, Hospital Indemnity Protection, Accident Indemnity Plan, Supplemental Dental and Group Legal Services).
New employees must enroll in the County's health benefit plans within thirty (30) days of the hire date.
The effective date of the health benefits coverage is the beginning of the month following a waiting period of forty-five (45) days from the date of hire.
After enrolling in the County's benefit plans, employees may only make changes to the plans either during the open enrollment period, which occurs annually (usually each October), and/or during the year, due to a family status change (i.e., marriage, births, divorce and adoption).
