Summary

The Population Health & Engagement RN promotes a culture of patient-centered care through service excellence and clinical quality driven by data. The Population Health & Engagement RN functions as an integral part of an interdisciplinary team to achieve optimal clinical outcomes through a seamless model of care with a focus on patient experience, quality clinical outcomes, and managing total cost of care. This person will assist with data analysis to assess patient experience, disease states, utilization, assist with care plan development and facilitation, referral routing to appropriate levels of care, care gap closure, etc. This person is responsible for leading the patient experience team and supporting a “people first” culture by guiding, implementing and coaching the strategic and integrated actions needed for consistently elevating safety and the patient experience-enterprise wide.

Responsibilities

  • Support the organization’s patient experience initiatives through direct interactions, effective omnichannel communications, and resolution of barriers to care by addressing the total individual (inclusive of medical, psychosocial, behavioral, and spiritual needs). Methods include, but are not limited to, rounding, answering patient calls, documenting in system of record and providing education to patients and loved ones.
  • Analyze patient survey data to provide quick and reoccurring feedback to collaborate with staff and leaders to identify solutions.
  • Help maintain patient flow and workflow by working collaboratively as a team member with staff to provide high quality and efficient service.
  • Leader to team members in the reporting structure including but not limited to hiring, schedules, timecards, and feedback.
  • Provide CTH caregivers with better understanding of the how's and whys of delivering patient-centric care, while coaching ways to improve outcomes and care processes.
  • Manage interface with communication software platforms, for effective leadership rounding on patients and staff, inpatient and ED discharge calls, point of care surveying (Kiosk), locational rounding to support quality, patient safety, and infection control.
  • Manage care concerns, formal grievances, patient complaints, utilizing a thorough investigation process.
  • Oversee HCAHPS data, support effective and sustainable plans to drive HCAHPS improvement.
  • Utilize innovative nursing and population management practices to support well-being throughout the continuum of care in order to promote quality, cost-effective outcomes. Participate in quality/performance improvement programs and projects, product evaluation, outcomes evaluation studies and/or clinical research.
  • Develop education programs as needed.
  • Performs other duties as assigned.

Qualifications

Required


  • Current State of Nevada licensure as Registered Nurse
  • Minimum of three (3) years’ experience in Clinical Care Setting
  • Two (2) years of leadership experience o Master’s in Population Health, Informatics, or equivalent experience
  • Compassionate, highly nuanced communicator equally able to inspire as to navigate conflict resolution with grace
  • Current American Heart Association (AHA) Basic Life Support (BLS)/Cardiopulmonary Resuscitation (CPR) for healthcare providers
Preferred
  • Minimum of five (5) years’ experience in Clincial Care Setting
  • Experience using Patient Engagement platforms (i.e. Cipher, PRC, etc)
  • Experience using Epic EMR
  • Experience with Patient Experience Surveys (HCAHPS, CGCAHPS, etc)
